加速康复管理在腹腔镜全子宫切除术患者中的应用研究

Application of accelerated rehabilitation management in patients with laparoscopic total hysterectomy

目的探讨加速康复管理在腹腔镜全子宫切除术患者中的应用效果。方法选取86例行腹腔镜全子宫切除术患者,根据随机数字表法分为对照组与观察组,每组43例。观察组采用加速康复管理,对照组采用常规护理。比较两组患者的手术时间、术中出血量、康复指标及并发症发生情况。结果两组患者的手术时间、术中出血量比较,差异无统计学意义(P>0.05)。观察组的术后首次排气时间、下床活动时间、住院时间分别为(22.5±6.7)h、(17.3±6.6)h、(4.4±0.8)d;对照组的术后首次排气时间、下床活动时间、住院时间分别为(26.1±5.8)h、(26.4±5.5)h、(5.8±1.1)d。观察组的术后首次排气时间、下床活动时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率低于对照组,但差异无统计学意义(P>0.05)。结论腹腔镜全子宫切除术患者采用加速康复管理,可促进患者术后康复,具有临床推广价值。

Objective To explore the effect of accelerated rehabilitation management in patients with laparoscopic total hysterectomy.Methods 86 laparoscopic total hysterectomy patients were selected and divided into control group and observation group according to random number table method,with 43 cases in each group.The observation group was treated with accelerated rehabilitation management,while the control group was treated with routine nursing.The operation time,intraoperative blood loss,rehabilitation indexes and complications were compared between the two groups.Results There was no significant difference in operation time and intraoperative blood loss between the two groups(P>0.05).In the observation group,the first postoperative exhaust time,the time of getting out of bed and the time of hospitalization were(22.5±6.7)h,(17.3±6.6)h and(4.4±0.8)d,respectively.In the control group,the first postoperative exhaust time,the time of getting out of bed and the time of hospitalization were(26.1±5.8)h,(26.4±5.5)h and(5.8±1.1)d,respectively.The first postoperative exhaust time,the time of getting out of bed and the time of hospitalization in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in observation group was lower than that in control group,but the difference was not statistically significant(P>0.05).Conclusion The use of accelerated rehabilitation management nursing for patients with laparoscopic total hysterectomy can promote postoperative rehabilitation of patients and has clinical popularization value.
