牙周炎行根面平整术患者术中应用超声洁牙机的效果观察

Observation on the Effect of Ultrasonic Dental Cleaner in Periodontitis Patients Undergoing Root Planing

目的:探讨在牙周炎患者实施根面平整术中应用超声洁牙机的效果。方法:选择2018年1月~2020年6月本院口腔科收治的80例牙周炎患者,随机分组,各40例,对照组单纯实施手工龈下刮除法联合根面平整术,观察组基于根面平整术联合超声龈下清创治疗,观察两组治疗前后牙周指数龈沟出血指数(SBI)、菌斑指数(PLI)、牙周袋深度(PD)以及临床附着丧失(CAL)等牙周组织健康状况,检测两组治疗前后龈沟液中肿瘤坏死因子-α(TNF-α)、单核细胞趋化蛋白-1(MCP-1)、基质金属蛋白酶8(MMP-8)等水平变化,观察两组临床治疗效果。结果:治疗前,两组SBI、PLI、PD、CAL水平无明显差异(P>0.05);治疗后,观察组SBI、PLI、PD、CAL水平较对照组低(P<0.05);治疗前,两组龈沟液中TNF-α、MCP-1、MMP-8水平无明显差异(P>0.05),治疗后,观察组龈沟液中TNF-α、MCP-1、MMP-8水平较对照组更低(P<0.05);观察组临床总有效率较对照组高(P<0.05)。结论:在牙周炎患者实施根面平整术中应用超声洁牙机的效果显著,不仅可以对牙周组织健康状况加以改善,还能抑制龈沟液中炎性细胞因子释放,继而控制牙周炎症,避免牙周组织损伤,促进病情康复。

Objective:To investigate the effect of ultrasonic dental cleaner in root planing of periodontitis patients.Methods:80 patients with periodontitis who were treated in our stomatology department from January 2018 to June 2020 were randomly divided into 40 patients each.The control group was treated with manual subgingival curettage combined with root planing.The observation group was treated with root planing combined with ultrasonic subgingival debridement.The periodontal index,sulcus bleeding index(SBI),plaque index(PLI)The periodontal pocket depth(PD),clinical attachment loss(CAL)and other periodontal tissue health conditions were measured,and tumor necrosis factor(TNF)in gingival crevicular fluid was detected before and after treatment in both groups-α(TNF-α),the levels of monocyte chemoattractant protein-1(MCP-1)and matrix metalloproteinase-8(MMP-8)were changed to observe the clinical therapeutic effects of the two groups.Results:There was no significant difference in the levels of SBI,PLI,PD and CAL between the two groups before treatment(P>0.05).After treatment,the levels of SBI,PLI,PD and CAL in the observation group were lower than those in the control group(P<0.05);Before treatment,there was no significant difference in the levels of TNF-α,MCP-1 and MMP-8 in gingival crevicular fluid between the two groups(P>0.05).After treatment,the levels of TNF-α,MCP-1 and MMP-8 in gingival crevicular fluid in the observation group were lower than those in the control group(P<0.05).The total clinical effective rate of the observation group was higher than that of the control group(P<0.05).Conclusion:The application of ultrasonic dental cleaner in root planing of periodontitis patients has a significant effect.It can not only improve the health of periodontal tissue,but also inhibit the release of inflammatory cytokines in gingival crevicular fluid,and then control periodontal inflammation,avoid periodontal tissue damage,and promote the recovery of the disease.
