地夸磷索钠滴眼液对超声乳化白内障吸除术后眼表的影响

The effects of diquafosol sodium eye drops on the ocular surface after phacoemulsification

目的评价应用3%地夸磷索钠滴眼液对超声乳化白内障吸除术眼表微环境的影响.方法前瞻性随机对照研究.随机选取郑州大学第一附属医院眼科2020年7月~10月进行超声乳化白内障吸除术的30例(60眼)作为研究对象.患者按随机数字表法分为两组:对照组(A组,30眼),术后进行常规局部抗炎治疗,并给予人工泪液滴眼6周;试验组(B组,30眼),术后常规局部抗炎治疗,应用人工泪液联合3%地夸磷索钠滴眼液6周.随访6周对比分析两组患者泪河高度、泪膜破裂时间、脂质层评分及睑板腺评分.结果滴眼2周比较两组泪河高度、首次/平均泪膜破裂时间、脂质层评分及睑板腺评分差异均无统计学意义(t=0.36、1.64、0.16、0.21、1.72,P=0.718、0.106、0.872、0.839、0.090),滴眼6周泪河高度、泪膜破裂时间及脂质层评分指标B组[(0.27±0.09)mm,(13.67±3.53)s,(3.93±0.91)分]优于A组[(0.22±0.07)mm,(11.58±3.40)s,(3.37±0.81)分](t=2.33、2.34、2.55,P=0.023、0.023、0.013),睑板腺评分两组差异无统计学意义[A组(6.67±1.09)分,B组(7.13±1.17)分](t=1.60,P=1.115).结论超声乳化白内障吸除术后应用3%地夸磷索钠滴眼液改善眼表疾病指数与泪液指标,在应用6周后对患者的泪河高度、泪膜破裂时间及脂质层评分表现出明显改善作用.

Objective Evaluation of the effect of the application of 3%diquafosol sodium eye drops on the ocular surface environment after cataract phacoemulsification.Methods This was a prospective randomized controlled study.Sixty eyes of 30 cases with cataract underwent phacoemulsification from Jul.to Oct.2020 in the First Affiliated Hospital of Zhengzhou University were randomly selected as study subjects.Patients were randomly divided into two groups.The control group received routine postoperative local anti-inflammatory treatment and artificial tear as group A.The trial group received routine postoperative local anti-inflammatory treatment and artificial tear combined with 3%diquafosol sodium for 6 weeks as group B.The tear meniscus height,tear breakup time,lipid layer score and lacrimal gland score of the two groups were compared during 6 weeks,follow-up.Results The differences in tear meniscus height,first/mean tear break-up time,lipid layer score and lacrimal gland score at 2 weeks were not statistically significant(t=0.36,1.64,0.16,0.21,1.72;P=0.718,0.106,0.872,0.839,0.090),and tear meniscus height,mean tear break-up time,and lipid layer score at 6 weeks were better in group B[(0.27±0.09)mm,(13.67±3.53)s,(3.93±0.91)score]than those in group A[(0.22±0.07)mm,(11.58±3.40)s,(3.37±0.81)scores](t=2.33,2.34,2.55;P=0.023,0.023,0.013).The difference in lacrimal gland score were not statistically significant[group A(6.67±1.09)scores,group B(7.13±1.17)scores](t=1.60,P=1.115).Conclusion Improvement of the ocular surface index and tear index with the application of 3%diquafoaol sodium eye drops after phacoemulsification gives significant improvement in the patients’tear meniscus height,tear break-up time,and lipid layer score after 6 weeks of application.
