D-二聚体和细胞角蛋白19片段在非小细胞肺癌老年患者中的表达

Expression of D-dimer and Cytokeratin 19 Fragments in Elderly Patients with Non-small Cell Lung Cancer

目的探讨D-二聚体(D-D)、细胞角蛋白19片段(CYFRA21-1)在非小细胞肺癌(NSCLC)患者中的表达情况。方法选取2017年4月~2019年4月在铁岭市中心医院治疗的150例NSCLC老年患者为观察组,同时选取健康老年体检者100例为对照组,检测外周血D-D、CYFRA21-1水平,分析其与患者肿瘤直径及临床分期等的关系。结果观察组血清D-D、CYFRA21-1水平高于对照组,差异有统计学意义(P<0.05)。肿瘤直径≥5 cm患者的血清D-D水平高于肿瘤直径<5 cm者,临床分期Ⅲ~Ⅳ期患者的血清D-D水平高于临床分期Ⅰ~Ⅱ期者,差异均有统计学意义(P<0.05)。临床分期Ⅲ~Ⅳ期患者的CYFRA21-1水平高于临床分期Ⅰ~Ⅱ期者,有淋巴结转移患者的CYFRA21-1水平高于无淋巴结转移者,差异均有统计学意义(P<0.05)。结论非小细胞肺癌患者外周血D-D、CYFRA21-1水平明显升高,D-D水平与肿瘤大小、临床分期有一定关系,CYFRA21-1水平与临床分期、是否淋巴转移有关。

Objective To investigate the expression of D-dimer(D-D)and cytokeratin 19 fragment(CYFRA21-1)in non-small cell lung cancer(NSCLC)patients.Methods A total of 150 elderly patients with NSCLC treated in Tieling Central Hospital from April 2017 to April 2019 were selected as the observation group,and 100 healthy elderly subjects were selected as the control group.The levels of D-D and CYFRA21-1 in peripheral blood were measured,and their relationship with tumor diameter and clinical stage was analyzed.Results The serum D-D and CYFRA21-1 levels in the observation group were higher than those in the control group,and the differences were statistically significant(P0.05).Patients with tumor diameter≥5 cm had higher serum D-D levels than those with tumor diameter5 cm,and patients with clinical stageⅢ-Ⅳhad higher serum D-D levels than those with clinical stage I-Ⅱ,and the differences were statistically significant(P0.05).CYFRA21-1 levels were higher in patients with clinical stagesⅢ-Ⅳthan in those with clinical stages I-Ⅱ,and higher in patients with lymph node metastasis than in those without lymph node metastasis,and the differences were statistically significant(P0.05).Conclusion The levels of D-D and CYFRA21-1 in peripheral blood of patients with non-small cell lung cancer were significantly increased,D-D levels were related to tumor size and clinical stage,and CYFRA21-1 levels were related to clinical stage and lymphatic metastasis.
