血清IgE、IFN-γ及Th17细胞因子水平与成人特应性皮炎病情严重程度的相关性

Correlation between serum levels of IgE,IFN‑gamma and Th17 cytokines and the severity of adult atopic dermatitis condition

目的:探讨成人特应性皮炎病情严重程度与机体血清免疫球蛋白E(IgE)、γ-干扰素(IFN-γ)及Th17细胞因子间的关系。方法:选取华中科技大学协和江北医院2018年9月—2019年9月收治的特应性皮炎患者80例,通过检测患者血清IgE、IFN-γ及Th17细胞因子[白细胞介素(IL)-17和IL-23]等水平,并分析与患者病情严重程度间的关系。结果:三组间血清IgE、IL-17、IL-23水平差异无统计学意义(P<0.05),且随着病情加重呈递增趋势;Spearman相关性分析显示血清IgE、IL-17、IL-23水平与成人特应性皮炎病情严重程度呈显著正相关(P<0.05);ROC曲线显示:IgE、IL-17、IL-23三项联合指标诊断轻中度成人特应性皮炎曲线下面积(AUC)为0.829,明显高于IgE、IL-23诊断的AUC(P<0.05),而诊断中重度成人特应性皮炎AUC为0.838,明显高于IL-17、IL-23诊断的AUC(P<0.05)。结论:成人特应性皮炎患者血清IgE、IL-17、IL-23水平呈现不同程度升高,并与患者的疾病轻重程度呈显著正相关,且这三项指标联合诊断对患者的病情具有较高的评估价值。

Objective:To investigate the relationship between the severity of atopic dermatitis in adults and serum levels of immunoglobulin E(IgE),interferon⁃gamma(IFN⁃γ),and Th17 cytokines.Methods:A total of 80 patients with atopic dermatitis admitted to Union Jiangbei Hospital of Huazhong Univer⁃sity of Science and Technology from September 2018 to September 2019 were selected.The serum levels of IgE,IFN⁃γ,Th17,interleukin(IL)⁃17,and IL⁃23,were detected,and the relationship be⁃tween the above indicators and the severity of the patient's disease was analyzed.Results:The levels of serum IgE,IL⁃17,and IL⁃23 were significantly different among the three groups in different dis⁃ease severity(P<0.05)and increased according to the severity of the disease.The spearman correlation analysis showed that serum IgE,IL⁃17,and IL⁃23 levels were positively correlated with the se⁃verity of adult atopic dermatitis(P<0.05).The ROC curve showed that the area under the curve(AUC)of IgE,IL⁃17,and IL⁃23 combined detection was 0.829 in the diagnosis of mild to moderate adult atopic dermatitis,which was significantly higher than that of IgE and IL⁃23 alone(P<0.05),while the AUC of combined detection in moderate to severe adult atopic dermatitis was 0.838,which was significantly higher than that of IL⁃17 and IL⁃23 alone(P<0.05).Conclusion:The levels of IgE,IL⁃17,and IL⁃23 in adult patients with atopic dermatitis increased to varying degrees and showed a significant positive correlation with the severity of the disease,and the combined diagnosis of these three indicators has a high evaluation value for the patients with atopic dermatitis.
