达格列净治疗老年慢性心力衰竭的效果及对患者认知功能的影响

Efficacy of dapagliflozin on elderly patients with chronic heart failure and its effect on cognitive function

目的:研究达格列净治疗老年慢性心力衰竭的临床效果及对患者认知功能的影响。方法:选取2020年1月至2022年1月住院治疗的110例老年慢性心衰患者作为研究对象,随机分为对照组(55例)和观察组(55例),对照组采用内科常规抗心衰治疗方案,观察组在常规治疗基础上加用达格列净,对两组患者的临床疗效和认知功能改善进行比较,记录不良反应发生情况。结果:治疗8周后,观察组左心室射血分数(LVEF)、心输出量(CO)、6 min步行距离(6MWD)均明显高于对照组(P<0.05),血清N末端脑钠肽前体(NT-proBNP)水平显著低于对照组(P<0.05);观察组蒙特利尔认知评价(MoCA)量表和简易智能状态检查(MMSE)量表的总得分均高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率差异无统计学意义(P>0.05)。结论:达格列净治疗老年慢性心衰患者效果显著,可有效改善患者心脏功能及认知功能,值得临床推广。

Objective:To study the clinical effect of dapagliflozin in the treatment of elderly patients with chronic heart failure and its influence on cognitive function.Methods:A total of 110 elderly patients with chronic heart failure who were hospitalized from January 2020 to January 2022 were included and were randomly divided into a control group(55 cases)and an observation group(55 cases).The con⁃trol group adopted conventional anti⁃heart failure therapy,and the observation group was treated with dapagliflozin on the basis of conventional treatment.The clinical efficacy and cognitive function im⁃provement of the two groups were compared,and adverse reactions were recorded.Results:After 8 weeks of treatment,the left ventricular ejection fraction(LVEF),cardiac output(CO)and 6⁃minute walking distance(6MWD)in the observation group were significantly higher than those in the control group(P<0.05);The total scores of Montreal Cognitive Assessment(MOCA)Scale and Mini⁃Men⁃tal State Examination(MMSE)in the observation group were significantly higher than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Dapagliflozin has a remarkable effect in the treatment of elder⁃ly patients with chronic heart failure,which can effectively improve the cardiac function and cognitive function of patients,and is worthy of clinical promotion.
