股神经阻滞超前镇痛缓解老年股骨骨折患者体位痛效应研究

Effect of Femoral Nerve Block Advanced Analgesia on Postural Pain in Elderly Patients with Femoral Fracture

目的:探讨股神经阻滞超前镇痛缓解老年股骨骨折患者体位痛效应。方法:选取我院2022年1月-2022年6月行单侧股骨骨折拟择股骨内固定手术患者150例,按数字随机表法分组,观察组以及对照组,各75例。对照组采用舒芬太尼超前镇痛,观察组采用股神经阻滞超前镇痛,观察比较2组镇痛效果。结果:入室时(TO)2组疼痛评分无明显差异(P>0.05),超前镇痛后10分钟后(T1)、腰硬麻醉转动体位时(T2),观察组疼痛评分明显低于对照组(P<0.05);观察组CI(心脏指数)、CO(心输出量)、SVV(每搏输出变异)指数较对照组高(P<0.05);观察组术后不良反应发生率明显低于对照组(P<0.05)。结论:股神经阻滞超前镇痛在老年股骨骨折中的运用效果理想,能有效缓解患者术中疼痛,稳定患者生命体征,减少药物不良反应,临床推广价值高。

Objective:To explore the effect of femoral nerve block advanced analgesia on postural pain in elderly patients with femoral fracture.Methods:A total of 150 patients who underwent unilateral femoral fracture pseudo-femoral internal fixation in Panyu Hospital of Traditional Chinese Medicine of Guangzhou City from January 2022 to June 2022 were selected and grouped according to numerical random table method,with 75 patients in each study group and reference group.Sufentanil advanced analgesia was used in the control group,and femoral nerve block advanced analgesia was used in the study group.The analgesia effects of the two groups were observed and compared.Results:There was no significant difference in pain score between the two groups(P>0.05)at home invasion(TO).The pain score of the study group was significantly lower than that of the control group 10 minutes after advanced analgesia(T1)and during lumbar hard anesthesia rotation(T2)(P<0.05).CI(cardiac index),CO(cardiac output)and SVV(stroke output variation)indexes in the study group were higher than those in the control group(P<0.05).The incidence of postoperative adverse reactions in the study group was significantly lower than that in the reference group(P<0.05).Conclusion:Femoral nerve block advanced analgesia has ideal effect in elderly patients with femoral fracture,which can effectively relieve intraoperative pain,stabilize vital signs,reduce adverse drug reactions,and has high clinical promotion value.
