上海市社区慢性病管理对象的共病现状及影响因素分析

Analysis of Community Comorbidity Status and Influencing Factors of Chronic Disease Management in Shanghai

目的了解上海市社区慢性病管理对象的共病现状,并分析其影响因素。方法采用上海市健康信息网市级平台数据,分析2304636名社区慢性病管理对象的高血压、糖尿病、慢性阻塞性肺病的基本信息及共病情况,并运用logistic回归探索其影响因素。结果调查对象中,患有1种慢性病的有1655469例(71.83%),患有2种慢性病的有576383例(25.01%),患有3种慢性病的有72784例(3.16%);2种及以上慢性病共病的比例为28.17%。不同性别、年龄、居住地、BMI情况、高血压和糖尿病服药情况患者的慢性病共病情况,差异具有统计学意义(P<0.05)。Logistic回归结果显示,居住地、性别、年龄、BMI、是否规律服用降压药和降糖药是慢性病共病的影响因素(P<0.05)。结论上海市社区慢性病管理对象存在共病情况,应进一步建立以人为核心的慢性病综合健康管理模式,重点关注男性及肥胖人群,展开针对性的预防措施。

Objective To understand the current situation of chronic diseases comorbidity of chronic diseases management objects in Shanghai community and analyze its influencing factors.Methods Based on the data of the municipal platform of Shanghai Health Information Network,the basic information and comorbidity of hypertension,diabetes and COPD in 2304636 community chronic disease management objects in Shanghai were analyzed,and the influencing factors were explored by logistic regression.Results There were 1655469 patients(71.83%)with one chronic disease;576383 patients(25.01%)with two chronic diseases;72784 patients(3.16%)with three chronic diseases;the proportion of two or more chronic diseases was 28.17%.There were significant differences in the comorbidity of chronic diseases among patients with different genders,age groups,areas,BMI,using drug for hypertension and diabetes(P<0.05).Logistic regression results showed that areas,gender,age,BMI,regular use of antihypertensive drugs and hypoglycemic drugs were the influencing factors of comorbidity chronic diseases(P<0.05).Conclusion There are comorbidities among the objects of chronic diseases management in Shanghai community,and a comprehensive health management model of chronic diseases with people as the core should be further established,focusing on men and obese people and launching targeted preventive measures.
