乳房下皱襞单切口微创旋切术治疗乳腺多发肿块的临床研究

Clinical study on the treatment of multiple breast masses by single incision minimally invasive rotary resection of inframammary crease

目的探讨乳房下皱襞单切口微创旋切术治疗乳腺多发肿块的临床效果。方法选取225例乳腺多发肿块患者作为研究对象,其中接受传统乳腺肿块微创旋切术治疗的105例为对照组,同期接受乳房下皱襞单切口微创旋切术治疗,且一般资料与对照组均衡可比的120例患者为观察组。比较两组患者的手术相关指标、术后疗效、术后并发症及切口美观满意度等指标。结果观察组手术时间、切口愈合时间短于对照组,视觉模拟评分法(VAS)评分低于对照组,差异均有统计学意义(均P<0.05)。两组术中出血量、临床疗效比较,差异均无统计学意义(均P>0.05)。观察组术后并发症发生率为5.83%,低于对照组的18.09%,差异有统计学意义(P<0.05)。观察组患者的切口美观满意度优于对照组,差异有统计学意义(P<0.05)。结论乳房下皱襞单切口微创旋切术治疗乳腺多发肿块,不仅能达到传统乳腺微创旋切术的效果,还能缩短手术时间,降低术后并发症发生率,以及提高切口美观满意度,具有临床推广价值。

Objective To investigate the clinical effect of single incision minimally invasive rotary resection of inframammary crease in the treatment of multiple breast masses.Methods A total of 225 patients with multiple breast masses were selected as the research object,among which 105 patients who treated with traditional minimally invasive rotary resection of breast masses were selected as the control group,and 120 patients who received single incision minimally invasive rotary resection of inframammary crease at the same time,and the general data was comparable with those of the control group were selected as the observation group.The operation related indexes,postoperative efficacy,postoperative complications and incision aesthetic satisfaction were compared between the two groups.Results The operation time and incision healing time of the observation group were shorter than those of the control group,and the visual analogue scale(VAS)score was lower than that of the control group,with statistically significant differences(all P<0.05).There was no significant difference in intraoperative blood loss and clinical efficacy between the two groups(all P>0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(5.83%vs.18.09%),with statistically significant difference(P<0.05).The incision aesthetic satisfaction of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Single incision minimally invasive rotary resection of inframammary crease in the treatment of multiple breast masses can not only achieve the effect of traditional minimally invasive rotary resection of breast masses,but also shorten the operation time,reduce the incidence of postoperative complications,and improve the incision aesthetic satisfaction,which has clinical popularization value.
