中药辨证联合Lucentis治疗糖尿病性黄斑水肿疗效观察

The clinical efficacy of traditional Chinese medicine combined Lucentis treatment on diabetic macular edema

目的评估中药辨证联合玻璃体腔内注射雷珠单抗(Lucentis)治疗糖尿病性黄斑水肿(DME)的疗效。方法选取糖尿病性黄斑水肿的患者36例(36眼),随机分为单纯玻璃体腔内注射Lucentis组(对照组)和玻璃体腔内注射Lucentis联合中药组(治疗组),观察1个月后两组患者治疗前和治疗后的最佳矫正视力(BCVA)、黄斑中心厚度(CMT)的变化以及1年内患者因糖尿病性黄斑水肿进行玻璃体腔注药的次数。结果对照组和治疗组治疗前后平均BCVA和平均CMT比较,差异有统计学意义(P<0.05)。治疗1个月后,对照组和治疗组黄斑区表现、平均BCVA和平均CMT比较,差异有统计学意义(P<0.05)。相较于对照组,治疗组1年内因糖尿病性黄斑水肿行玻璃体腔注药的次数少,差异有统计学意义(P<0.05)。结论中西医结合治疗DME,将中医整体观念与西医局部治疗相联系,相辅相成,可以加快黄斑水肿的吸收,缩短疗程,提高视力,改善全身情况。

Objective To evaluate the clinical efficacy of traditional Chinese medicine combined Lucentis treatment on diabetic macular edema(DME).Methods Thirty-six patients(36 eyes)with diabetic macular edema was selected.Patients were randomly divided into two groups:intravitreal injection of Lucentis group(control group)and intravitreal injection of Lucentis combined with traditional Chinese medicine group(treatment group).The changes of best-corrected visual acuity(BCVA)and central macular retinal thickness(CMT)of the two groups before and after treatment were observed one month later,as well as the number of needles for intravitreal injection because of DME within one year.Results The differences of all the patients'BCVA and CMT were statistically significant between before and after treatment(P<0.05).After one month of treatment,the differences between control group and treatment group in macular manifestation,BCVA and CMT were statistically significant(P<0.05).Compared to control group,patients in treatment group received less injections in one year and the results were statistically significant(P<0.05).Conclusion Traditional Chinese medicine combined with Lucentis treatment for diabetic macular edema can accelerate the absorption of macular edema,shorten the period of treatment,improve vision,and improve the general condition.The whole concept of traditional Chinese medicine and western medicine topical treatment was linked.
