炎症性肠病病人疾病管理能力影响因素研究

Study on influencing factors of disease management ability in patients with inflammatory bowel disease

目的:探讨炎症性肠病病人疾病管理能力的影响因素。方法:选取2017年6月-2022年6月于山东大学齐鲁医院(青岛)消化内科住院的510例炎症性肠病病人作为研究对象,采用自行设计的一般资料调查问卷、家庭韧性评估量表、慢性病资源问卷、慢性疾病管理能力评估量表等对病人进行问卷调查,查找影响炎症性肠病病人疾病管理能力的因素。结果:单因素分析显示,年龄、文化程度、居住地、吸烟史、饮酒史、性格特质、睡眠状况、共病指数及疾病严重程度是炎症性肠病病人疾病管理能力的影响因素(P<0.05);相关性分析显示,炎症性肠病病人疾病管理能力与家庭韧性各个维度得分及总分呈显著正相关(r值为0.552~0.989,P<0.01);与慢性病资源利用各个维度得分及总分呈显著正相关(r值为0.639~0.759,P<0.01);多因素分析结果显示,年龄、文化程度、饮酒史、性格特质、睡眠状况、共病指数、疾病严重程度、家庭韧性及慢性病资源利用是炎症性肠病病人疾病管理能力的影响因素(P<0.05)。结论:年龄在18~30岁、小学及以下文化程度、有饮酒史、情绪不稳、睡眠存在障碍、共病指数高、疾病处于重度活动期、家庭韧性低及慢性病资源利用差的炎症性肠病病人疾病管理能力低,医护人员应着重关注此类病人,制订针对性的干预措施,提高其疾病管理能力。

Objective:To explore the influencing factors of disease management ability in patients with inflammatory bowel disease.Methods:A total of 510 patients with inflammatory bowel disease who were hospitalized in the gastroenterology department of Qilu Hospital of Shandong University(Qingdao)from June 2017 to June 2022 were selected as the research objects.Self-designed general data questionnaire,Family Resilience Assessment Scale,Chronic Illness Resource Survey,Chronic Disease Management Ability Scale,Pittsburgh Sleep Quality Index,Charlson Comorbidity Index and Eysenck Personality Questionnaire were used to investigate the patients.To identify factors that influence disease management ability in patients with inflammatory bowel disease.Results:Univariate analysis showed that age,education level,place of residence,smoking history,drinking history,personality traits,sleep status,comorbidity index and disease severity were the influencing factors of disease management ability of IBD patients(P<0.05).Correlation analysis showed that the disease management ability of IBD patients was significantly positively correlated with the scores of all dimensions and total scores of family toughness(r values were 0.552~0.989,P<0.01).There was a significant positive correlation between the scores of each dimension and the total score of chronic disease resource utilization(r value 0.639~0.759,P<0.01).The results of multivariate analysis showed that age,education,drinking history,personality traits,sleep status,comorbidity index,disease severity,family toughness and chronic disease resource utilization were the factors influencing the disease management ability of IBD patients(P<0.05).Conclusions:Inflammatory bowel disease patients aged 18~30 years old,primary school education or below,history of alcohol consumption,emotional instability,sleep disorders,high comorbidity index,severe disease activity,low family resilience and poor utilization of chronic disease resources have low disease management ability.Healthcare workers should pay attention to these patients and formulate targeted intervention measures to improve their disease management ability.
