β-羟基-β-甲基丁酸辅助治疗梅花鹿颈椎骨折病例分析

摘要 在野外及自然保护区中梅花鹿出现骨折的病例较多,骨折后的致死率和致残率较高,该类疾病严重影响了梅花鹿的生活质量、野外救治及人工养殖场的经济效益。2022年8月7日,黑龙江省某自然保护区的1只3月龄梅花鹿出现四肢瘫痪症状,通过X光检测确诊为颈椎骨折。根据其个体情况和骨折状况,采取食谱调整、限制运动、学步车辅助治疗、药物注射等中西医结合治疗方案,其中在食谱调整中重点补充β-羟基-β-甲基丁酸(β-hydroxy-β-methylbutyric acid,HMB)以促进肌肉蛋白合成,增加肌肉力量及耐力。治疗3周后幼鹿四肢反射渐进性恢复,疼痛感增强,肌肉紧张度较好松弛度较低。2个月后,幼鹿偶尔可以自主尝试站立,学步车内尝试正常走路的步伐增多,前后肢肌肉弹性增加。3个月后,幼鹿颈部活动自由,可以独立站立行走跑动,双侧肌肉对称,肌肉的神经反射正常。4个月后,幼鹿起卧自主,运动状态正常,放回室外保护区内自然饲养。论文记录了通过补充HMB和中西医结合治疗梅花鹿颈椎骨折从瘫痪到康复的救治过程,以期为野生动物颈椎骨折疾病的诊治提供新思路,为HMB在野生动物相关疾病中的应用提供参考。 In the wild and in nature reserves,there are many cases of fractures in Cervus nippon,and the death and disability rates after fractures are high.This kind of disease seriously affects the quality of life of Cervus nippon,field treatment and economic benefits of artificial farms.On August 7,2022,a 3-month-old Cervus nippon in a nature reserve in Heilongjiang province developed symptoms of quadriplegia and was diagnosed with a cervical spine fracture through X-ray examination.According to the fracture status of Cervus nippon,the diet adjustment,restrict exercise,baby walker auxiliary treatment,drug injection and other integrated traditional Chinese and Western medicine treatment plan were conducted,in which diet adjustment focuses on supplementingβ-hydroxy-β-methylbutyric acid(β-Hydroxy-β-methylbutyric acid,HMB)to promote muscle protein synthesis,increase muscle strength and endurance.After three weeks of treatment,the reflexes of the limbs of the young Cervus nippon gradually recovered,the pain increased,and the muscle tension was better and the relaxation was lower.After two months,the young Cervus nippon can occasionally try to stand independently,the pace of normal walking in the toddler car increased,and the muscle elasticity of the front and rear limbs increased.After three months,the neck of the young Cervus nippon can move freely and can stand,walk and run independently.The muscles on both sides were symmetrical and the nerve reflex of the muscles was normal.After four months,the young Cervus nippon got up and down autonomously,and the state of movement was normal,so it was put back into the outdoor protection area to raise naturally.This article records the treatment process of Cervus nippon cervical spine fractures from paralysis to rehabilitation by supplementing HMB and integrated traditional Chinese and Western medicine treatment,in order to provide new ideas for the diagnosis and treatment of cervical spine fractures in wild animals and provide reference for the application of HMB in related diseases of wild animals.
