首次化疗的胃癌患者焦虑现状及其影响因素

The status of anxiety in gastric cancer patients undergoing first chemotherapy and its influencing factors

目的探讨胃癌首次化疗患者焦虑情绪现状及影响因素。方法本研究纳入2019年12月~2023年6月88例胃癌患者为研究对象,化疗1个周期结束后进行焦虑筛查,根据筛查结果设为焦虑组以及正常组。比较两组患者年龄、性别、居住地等一般资料,采用logistic回归分析调查胃癌首次化疗患者焦虑情绪的影响因素。结果88例患者合并焦虑35例,轻度12例,中度18例,重度5例;焦虑组与正常组居住地、肿瘤分期、不良反应、性别、文化程度、是否手术差异具有统计学意义(P<0.05),两组年龄、医疗支付类型、婚姻情况差异无统计学意义(P>0.05);logistic回归分析显示,居住农村、肿瘤分期Ⅲ~Ⅳ期、不良反应Ⅳ级、女性、小学文化程度是胃癌首次化疗患者焦虑情绪的影响因素(P<0.05)。结论胃癌首次化疗患者焦虑情绪较为常见,以轻中度焦虑为主,居住农村、中晚期胃癌等是影响患者焦虑情绪的主要因素。

Objective To investigate the current status and influencing factors of anxiety in patients undergoing first chemotherapy for gastric cancer.Methods This study included 88 gastric cancer patients from December 2019 June 2023,who were screened for anxiety after 1 cycle of chemotherapy,and according to the screening results,they were divided into the anxiety group and the normal group.General data such as age,gender,and place of residence were compared between the two groups,and logistic regression analysis was used to investigate the factors affecting anxiety in patients with gastric cancer undergoing first chemotherapy.Results There were 35 cases of comorbid anxiety disorders in 88 patients,12 cases of mild,18 cases of moderate,and 5 cases of severe;the differences between the anxiety group and the normal group in terms of place of residence,tumor stage,adverse reaction,gender,education level,and whether or not they were operated were statistically significant(P<0.05).The differences in age,type of medical payment,and marital status between the two groups were not statistically significant(P>0.05).logistic regression analysis showed that residence in rural areas,tumor stage III-IV,adverse reaction grade IV,female,and elementary school literacy were the influencing factors of anxiety in patients undergoing first chemotherapy for gastric cancer(P<0.05).Conclusion Anxiety is more common in patients undergoing first chemotherapy for gastric cancer,with mild-to-moderate anxiety predominating,and living in a rural area and middle and late stage gastric cancer are the main factors affecting patients'anxiety.
