脑白质病变患者磁共振弥散张量成像及脑电图诊断结果与认知功能的相关性

Correlation between MRI Diffusion Tensor Imaging and Electroencephalogram Diagnostic Results and Cognitive Function in Patients with White Matter Lesions

摘要 目的:分析脑白质病变(WML)患者磁共振弥散张量成像、脑电图诊断结果与认知功能的相关性。方法:选取2021年9月—2022年12月南京市浦口区中医院行头颅MRI者160例为研究对象,健康者为对照组(40例),根据蒙特利尔认知评估量表(MoCA)评分将WML患者分为WML伴认知正常组、WML伴轻度认知障碍组、WML伴中重度认知障碍组,各40例。比较四组MoCA评分、不同大脑区域相对功率比(DTABR)、各向异性(FA)、平均扩散系数(MD)值。结果:WML伴轻度认知障碍组、WML伴中重度认知障碍组MoCA评分低于对照组、WML伴认知正常组,WML伴轻度认知障碍组、WML伴中重度认知障碍组各大脑区域DTABR低于对照组、WML伴认知正常组,WML伴轻度认知障碍组、WML伴中重度认知障碍组各大脑区域MD高于对照组、WML伴认知正常组,WML伴轻度认知障碍组、WML伴中重度认知障碍组各大脑区域FA低于对照组、WML伴认知正常组,差异均有统计学意义(P<0.05);不同区域DTABR值与MoCA评分呈正相关(P<0.05)。结论:磁共振弥散张量成像联合脑电图测定与WML患者的认知功能具有相关性。 Objective:To analyze the correlation between MRI diffusion tensor imaging and electroencephalogram diagnostic results and cognitive function in patients with white matter lesions(WML).Methods:A total of 160 patients who underwent head MRI were selected as the study subjects in the Nanjing Pukou District Hospital of Traditional Chinese Medicine from September 2021 to December 2022,and 40 healthy subjects were selected as the control group.According to MoCA scores,WML patients were divided into WML group with normal cognition,WML group with mild cognitive impairment,and WML group with moderate to severe cognitive impairment,with 40 cases in each group.MoCA scores,different brain area relative power ratios(DTABR),anisotropy(FA)and mean diffusion coefficient(MD)were compared among the four groups.Results:The MoCA scores of the WML with mild cognitive impairment group and the WML with moderate to severe cognitive impairment group were lower than those of the control group and the WML with normal cognition group,the DTABR of each brain region of WML with mild cognitive impairment and WML with moderate to severe cognitive impairment were lower than those of control group and WML with normal cognition,the MD of each brain region in the WML with mild cognitive impairment group and the WML with moderate to severe cognitive impairment group was higher than that in the control group and the WML with normal cognition group,the FA of each brain region in the severe cognitive impairment group and the WML with moderate to severe cognitive impairment group was lower than that in the control group and the WML with normal cognition group,and the differences were statistically significant(P<0.05);DTABR values in different regions were positively correlated with MoCA scores(P<0.05).Conclusion:MRI diffusion tensor imaging combined with electroencephalogram measurement is related to the cognitive function of WML patients.
