加速康复外科在老年结直肠癌患者的应用可行性及安全性研究

The feasibility and safety of enhanced recovery after surgery in elderly patients with colorectal cancer

摘要 目的探讨加速康复外科理念在老年结直肠癌患者中的应用价值。方法回顾性分析74例大于70岁结直肠癌患者资料,按围手术期管理方式不同分为实验组(n=37)和对照组(n=37),实验组围手术期采用ERAS管理理念,对照组采用传统的围手术期管理方案。比较两组患者一般临床资料、术中及术后恢复、疼痛评分及并发症发生率等情况。结果与对照组相比,ERAS组患者术后首次下地活动时间、首次进食时间、排气时间、尿管、引流管拔除时间更早,术后1d、4d疼痛评分更低,术后住院时间更短,差异有统计学意义。术后并发症发生率无明显差异。结论ERAS理念在老年结直肠癌围手术期中的应用能加速患者康复,缩短住院时间,安全有效。 Objective To evaluate the value of enhanced recovery after surgery(ERAS)in elderly patients with colorectal cancer(CRC).Methods The clinical data of 74 patients with colorectal cancer over 70 years were analyzed retrospectively.According to the perioperative treatment method,the patients were divided into experimental group(ERAS treatment,n=37)and the control group(traditional treatment,n=37).The general clinical data,intraoperative and postoperative recovery,pain score,the incidence of complications and hospitalization were compared between the two groups.Results Compared with the control group,the ERAS group patients had earlierfirst ambulationtime,first meal time,exhausttime,urinary catheter and drainage tube removal time,and lower pain score on the lst and 4th day after operation,postoperative hospital stay was shorter,the difference was statistically significant.There was no significant difference in the incidence of postoperative complications.Conclusion The application of ERAS in the perioperative period of colorectal cancer in the elderly can accelerate the recovery of patients,shorten the length of hospital stay.
