拉莫三嗪联合奥卡西平、托吡酯治疗癫痫患儿的效果及对睡眠质量的影响

Effect of lamotrigine combined with oxcarbazepine and topiramate in the treatment of children with epilepsy and its influence on sleep quality

目的探讨拉莫三嗪联合奥卡西平、托吡酯治疗癫痫患儿的临床效果。方法选取2018年5月至2021年5月本院收治的50例癫痫患儿作为研究对象,根据不同治疗方案将其分为对照组(24例,奥卡西平+托吡酯治疗)和研究组(26例,奥卡西平+托吡酯+拉莫三嗪治疗)。比较两组的治疗效果。结果研究组的治疗总有效率高于对照组(P<0.05)。治疗后,研究组的超敏C反应蛋白(hs-CRP)、同型半胱氨酸(Hcy)、神经元特异性烯醇化酶(NSE)水平均明显低于对照组(P<0.05)。治疗后,两组的δ、θ、α、β波频率及癫痫样放电频率均低于治疗前,且研究组低于对照组(P<0.05)。治疗后,研究组的入睡潜伏期、浅睡眠期、深睡眠期及快速眼动睡眠期优于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论拉莫三嗪联合奥卡西平、托吡酯治疗癫痫患儿的效果显著,可控制脑电活动异常变化,调节机体炎症因子和神经损伤因子水平,进而改善患儿的睡眠质量,值得推广。

Objective To explore the clinical effect of lamotrigine combined with oxcarbazepine and topiramate in the treatment of children with epilepsy.Methods A total of 50 children with epilepsy admitted in our hospital from May 2018 to May 2021 were selected as the research objects.According to different treatment schemes,the children were divided into control group(24 cases,oxcarbazepine+topiramate treatment)and study group(26 cases,oxcarbazepine+topiramate+lamotrigine treatment).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).After treatment,the levels of high-sensitivity C-reactive protein(hs-CRP),homocysteine(Hcy)and neuronspecific enolase(NSE)in the study group were significantly lower than those in the control group(P<0.05).After treatment,the frequencies ofδ,θ,α,βwaves and epileptiform discharges in the two groups were lower than those before treatment,and those in the study group were lower than the control group(P<0.05).After treatment,the sleep latency,shallow sleep period,deep sleep period and rapid eye movement sleep period of the study group were better than those of the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Lamotrigine combined with oxcarbazepine and topiramate in the treatment of children with epilepsy has a significant effect,which can control the abnormal changes of brain electrical activity,regulate the levels of inflammatory factors and nerve injury factors,and improve the sleep quality of children,it is worth promoting.
