乙酰半胱氨酸加布地奈德雾化吸入对肺炎患儿的疗效和肺功能的影响研究

Study on the Effect of Acetylcysteine Plus Budesonide Nebulized Inhalation on the Efficacy and Lung Function of Children with Pneumonia

摘要 目的评估乙酰半胱氨酸+布地奈德雾化吸入应用在肺炎患儿中的疗效及对肺功能的影响。方法方便选取2021年3月—2022年7月武威市凉州医院收治的86例肺炎患儿为研究对象,按照随机数表法分为对照组(43例,行布地奈德雾化吸入治疗)、观察组(43例,在对照组基础上加用乙酰半胱氨酸),评价组间治疗有效率、肺功能、炎性因子、心肌酶谱指标。结果观察组治疗有效率为95.35%高于对照组的76.74%,差异有统计学意义(χ^(2)=6.198,P<0.05)。治疗1周后,观察组第1秒最大呼气量(maximum expiratory volume in the first second,FEV1)、最高呼气峰流速(peak expiratory flow,PEF)、用力肺活量(forced vital capacity,FVC)水平高于对照组,差异有统计学意义(P<0.05)。治疗1周后,观察组降钙素原(procalcitonin,PCT)、白细胞介素-6(interleukin-6,IL-6)、C反应蛋白(C-reactive protein,CRP)水平低于对照组,差异有统计学意义(P<0.05)。治疗1周后,观察组肌酸激酶同工酶(creatine kinase isoenzyme,CK-MB)、肌酸激酶(creatine kinase,CK)、乳氨酸脱氢酶(lactine dehydrogenase,LDH)水平低于对照组,差异有统计学意义(P<0.05)。结论对肺炎患儿采用乙酰半胱氨酸+布地奈德雾化吸入,可以提高治疗效果,减轻炎症状态及心肌损伤,改善肺功能。 Objective To assess the efficacy of acetylcysteine+budesonide nebulized inhalation application in children with pneumonia and the effect on lung function.Methods 86 children with pneumonia admitted to Liangzhou Hospital in Wuwei City from March 2021 to July 2022 were conveniently selected as the study subjects.They were divided into a control group(43 cases receiving budesonide nebulization inhalation treatment)and an observation group(43 cases receiving acetylcysteine on the basis of control group)using a random number table method.The treatment efficacy,lung function,inflammatory factors,and myocardial enzyme spectrum indicators were evaluated between the groups.Results The effective rate of treatment in the observation group(95.35%)was higher than that in the control group(76.74%),the difference was statistically significant(χ^(2)=6.198,P<0.05).After one week of treatment,the levels of maximum expiratory volume in the first second(FEV1),peak expiratory flow(PEF),forced vital capacity(FVC)in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).After one week of treatment,the levels of procalcitonin(PCT),interleukin-6(IL-6),C-reactive protein(CRP)in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).After one week of treatment,the levels of creatine kinase isoenzyme(CK-MB),creatine kinase(CK),lactine dehydrogenase(LDH)in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion The use of acetylcysteine+budesonide nebulized inhalation for children with pneumonia can improve the therapeutic effect,reduce the inflammatory state and myocardial injury,and improve lung function.
