乳腺癌改良根治术与保乳手术治疗乳腺癌对血清肿瘤标志物的影响比较

Comparison of the Effects of Modified Radical Mastectomy and Breastconserving Surgery on Serum Tumor Markers in the Treatment of Breast Cancer

目的探讨乳腺癌根治术与保乳手术在乳腺癌中对血清肿瘤标志物的影响。方法随机选取2020年1月—2023年7月泗阳县中医院接诊的40例乳腺癌手术患者,按照随机数字表法将其分成对照组和观察组,各20例。对照组患者以乳腺癌改良根治术进行治疗,观察组患者则接受保乳手术治疗。对比两组患者手术效果。结果观察组手术用时、术中出血量、术后平均引流量以及住院时间均显著优于对照组,差异有统计学意义(P<0.05)。观察组术后美容优良率(95.00%)较对照组(70.00%)更高,差异有统计学意义(χ^(2)=4.329,P<0.05)。术后,观察组血清中糖类抗原15-3和癌胚抗原水平显著低于对照组,差异有统计学意义(P<0.05)。两组局部复发率和远处转移率比较,差异无统计学意义(P>0.05)。结论保乳手术的应用可以提高乳腺癌患者治疗结果以及术后美容效果,手术安全性更高,对患者血清肿瘤标志物水平影响较小,手术方式值得推广。

Objective To explore the effects of radical mastectomy and breast-conserving surgery on serum tumor markers in breast cancer.Methods From January 2020 to July 2023,40 patients with breast cancer who were treated in Siyang County Hospital of Traditional Chinese Medicine were randomly selected and divided into a control group and an observation group according to the random number table method,with 20 cases in each group.The patients in the control group were treated with modified radical mastectomy for breast cancer,while the patients in the observation group were treated with breast conserving surgery.Compared the surgical outcomes between two groups of patients.Results The observation group had significantly better surgical time,intraoperative bleeding volume,average postoperative drainage volume,and hospital stay than the control group,the difference was statistically significant(P<0.05).The observation group had higher postoperative cosmetic excellence rate(95.00%)compared to the control group(70.00%),the difference was statistically significant(χ^(2)=4.329,P<0.05).After surgery,the levels of carbohydrate antigens 15-3 and carcinoembryonic antigens in the serum of the observation group were significantly lower than those of the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the local recurrence rate and distant metastasis rate between the two groups(P>0.05).Conclusion The application of breast-conserving surgery can improve the therapeutic outcome of breast cancer patients as well as the postoperative cosmetic effect,with higher surgical safety and less impact on the level of serum tumor markers of patients,and the surgical method is worth promoting.
