探究控制性减压术对老年颅脑外伤患者疗效及术后转归的影响

Exploring the Effect of Controlled Decompression on the Efficacy and Postoperative Regression of Elderly Patients with Craniocerebral Trauma

目的探究控制性减压术对老年颅脑外伤患者疗效及术后转归的影响。方法将2013年1月—2022年12月在江苏省句容市人民医院接受治疗的老年颅脑外伤患者113例纳入本研究,按照随机数表法分为研究组(n=56)、对照组(n=57),分别行控制性减压术、标准外伤大骨瓣开颅减压术,对比两组患者治疗效果与并发症发生率。结果相较于对照组,研究组血压骤降(1.79%)、急性脑膨出(10.71%)、脑梗死(3.57%)、脑积水(7.14%)、迟发性脑水肿(3.57%)发生率均更低,差异有统计学意义(χ^(2)=4.730、4.544、8.863、4.497、4.799,P<0.05);与对照组相比较,研究组临床治疗总有效率(94.64%)更高,差异有统计学意义(χ^(2)=9.266,P<0.05)。结论老年颅脑外伤患者治疗中应用控制性减压术,可降低并发症发生率,提高疗效。

Objective To investigate the effect of controlled decompression on the efficacy and postoperative regression of elderly patients with craniocerebral trauma.Methods 113 elderly patients with craniocerebral trauma treated in Jurong People's Hospital from January 2013 to December 2022 were included in this study,and according to the randomized numerical table method divided into the study group(n=56)and the control group(n=57),and were subjected to controlled decompression and standard traumatic craniectomy with large bone flap,respectively.The treatment effect and complication rate were compared between the two groups.Results Compared with the control group,the study group had a lower incidence of blood pressure plunge(1.79%),acute cerebral bulging(10.71%),cerebral infarction(3.57%),hydrocephalus(7.14%),and delayed cerebral edema(3.57%),and the difference was statistically significant(χ^(2)=4.730,4.544,8.863,4.497,4.799,P<0.05).Compared with the control group,the total effective rate of clinical treatment(94.64%)was higher in the study group,and the difference was statistically significant(χ^(2)=9.266,P<0.05).Conclusion The application of controlled decompression in the treatment of elderly patients with craniocerebral trauma can reduce the complication rate and improve the efficacy.
