柔肝调神针刺法治疗肝郁血虚型脑卒中后睡眠障碍临床观察

Softening Liver and Regulating Spirit Acupuncture in the Treatment of Post-stroke Sleep Disorders of Liver Stagnation and Blood Deficiency Type

目的观察柔肝调神针刺法治疗肝郁血虚型脑卒中后睡眠障碍的临床效果。方法收集60例肝郁血虚型脑卒中后睡眠障碍患者,随机分为治疗组和对照组,各30例。治疗组在基础治疗的同时予以柔肝调神针刺法治疗,对照组在基础治疗的同时予以口服艾司唑仑片治疗。2组均采用匹兹堡睡眠质量指数量表(PSQI)、脑卒中专用生活质量量表(SS-QOL)和汉密尔顿抑郁量表(HAMD)进行评价,并比较疗效差异。结果治疗期间,治疗组有2例患者因出现晕针现象主动要求退出,其余患者均完成治疗。1个疗程后,治疗组总有效率为92.86%(26/28),高于对照组的80.00%(24/30),差异有统计学意义(P<0.05);2组PSQI评分、SS-QOL评分和HAMD评分均有改善,且治疗组的各项功能评分明显优于对照组(P<0.05)。结论柔肝调神针刺法治疗肝郁血虚型脑卒中后睡眠障碍效果明显,能有效缓解患者睡眠障碍症状,提高患者生活质量,改善患者抑郁情绪,值得临床推广。

Objective To observe the clinical efficacy of softening liver and regulating spirit acupuncture in the treatment of post-stroke sleep disorders of liver stagnation and blood deficiency type.Methods A total of 60 patients with post-stroke sleep disorders of liver stagnation and blood deficiency type were randomly divided into treatment group and control group,with 30 cases in each group.The treatment group was treated with softening liver and regulating spirit acupuncture at the same time of basic treatment,and the control group was treated with oral Esazolam tablets at the same time of basic treatment.Pittsburgh Sleep Quality Index Scale(PSQI),Stroke Specific Quality of Life Scale(SS-QOL)and Hamilton Depression Scale(HAMD)were used to evaluate the two groups,and the differ⁃ence in efficacy was compared.Results During the treatment period,2 patients in the treatment group voluntarily asked to withdraw due to the phenomenon of needle fainting,and the other patients completed the treatment.After one course of treatment,the total effective rate of the treatment group was 92.86%(26/28),which was higher than that of the control group(80.00%,24/30),and the difference was statistically significant(P<0.05).PSQI scores,SS-QOL scores and HAMD scores were improved in both groups,and the functional scores of the treatment group were significantly better than those of the control group(P<0.05).Conclusion Softening liver and regulat⁃ing spirit acupuncture has obvious curative effect on post-stroke sleep disorders of liver stagnation and blood deficiency type,can effec⁃tively relieve patients’sleep disorder symptoms,improve patients’life treatment and depression,and is worthy of clinical application.
