环孢素联合低分子肝素钙治疗原因不明复发性流产可行性分析

目的分析环孢素联合低分子肝素钙治疗原因不明复发性流产的效果。方法选取2019年5月至2022年12月江西省妇幼保健院收治的60例原因不明复发性流产患者作为研究对象,按照随机数字表法分为治疗组(30例)与对照组(30例)。对照组采用低分子肝素钙治疗,治疗组在对照组基础上加用环孢素。比较两组患者的临床疗效、妊娠结局(足月分娩、异位妊娠及自然流产)、CD4^(+)/CD8^(+)水平及人绒毛促性腺激素水平。结果治疗组患者的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗组的成功妊娠率高于对照组,差异有统计学意义(P<0.05)。治疗组治疗后的CD4^(+)/CD8^(+)低于对照组,治疗1、2、3周的人绒毛促性腺激素水平高于对照组,差异有统计学意义(P<0.05)。结论环孢素联合低分子肝素钙可提高原因不明复发性流产患者血人绒毛促性腺激素,降低CD4^(+)/CD8^(+),促进滋养细胞及孕囊、胚芽的生长,可提高保胎成功率。

Objective To analyze the effect of Ciclosporin combined with Low Molecular Weight Heparin Calcium in the treatment of unexplained recurrent abortion.Methods A total of 60 patients with unexplained recurrent abortion admitted to Jiangxi Maternal and Child Health Hospital from May 2019 to December 2022 were selected as research objects,and they were divided into treatment group(30 cases)and control group(30 cases)according to random number table method.The control group was treated with Low Molecular Weight Heparin Calcium,and the treatment group was supplemented with Ciclosporin on the basis of control group.The clinical efficacy,pregnancy outcome(full-term delivery,ectopic pregnancy and spontaneous abortion),CD4^(+)/CD8^(+)levels and human villus gonadotropin levels were compared between the two groups.Results The total effective rate of treatment group was higher than that of control group,the difference was statistically significant(P<0.05).The successful pregnancy rate in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).The CD4^(+)/CD8^(+)after treatment in the treatment group was lower than that in the control group,and the human villus gonadotropin level in the treatment group was higher than that in the control group at 1,2 and 3 weeks,the differences were statistically significant(P<0.05).Conclusion Ciclosporin combined with Low Molecular Weight Heparin Calcium can increase the human villus gonadotropin,reduce CD4^(+)/CD8^(+),promote the growth of trophoblast,gestational capsule and germ in patients with unexplained recurrent abortion,and improve the success rate of fetal protection.
