多种油脂肪乳对极低出生体重儿肝功能、血脂的影响分析

The Effect of Multiple Oil and Fat Emulsions on Liver Function and Blood Lipids in Very Low Birth Weight Infants

目的探讨多种油脂肪乳(SMOF)在极低出生体重(VLBW)儿治疗中的运用效果。方法选取2018年1月至2022年9月在我院NICU治疗的200例VLBW儿,依据不同住院时间段分为对照组和观察组,2018年1月至2020年5月住院应用中长链脂肪乳剂(MCT/LCT)的100例VLBW儿为对照组,2020年6月至2022年9月住院应用SMOF的100例VLBW儿为观察组。比较两组肝功能、血脂指标、并发症发生率。结果胃肠外营养(PN)治疗前,两组肝功能、血脂指标比较,无显著差异(P>0.05);PN治疗后,观察组血清总胆红素(TBIL)、血清直接胆红素(DBIL)、丙氨酸氨基转移酶(ALT)、γ-谷氨酰转肽酶(γ-GT)、碱性磷酸酶(ALP)、胆汁酸(TBA)、血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)峰值均低于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论SMOF可减少VLBW儿并发症的发生,减少对肝功能与血脂的影响,利于身体恢复。

Objectives To explore the application effect of multiple oil fat emulsions(SMOF)in the treatment of very low birth weight(VLBW)infants.Methods A total of 200 VLBW infants who were treated in NICU of our hospital from January,2018 to September,2022 were selected and divided into control group and observation group according to different hospitalization time.The 100 VLBW infants who were hospitalized from January,2018 to May,2020 using medium and long chain fat emulsion(MCT/LCT)were selected as the control group,and the 100 VLBW infants who were hospitalized from June 1,2020 to September 30,2022 using SMOF were selected as the observation group.The liver function,blood lipid indicators,and incidence of complications were compared between the two groups.Results There was no significant difference in liver function and blood lipid indicators between the two groups before parenteral nutrition(PN)treatment(P>0.05).After PN treatment,the peak values of serum total serum total bilirubin(TBIL),serum direct bilirubin(DBIL),alanine aminotransferase(ALT)γ-Glutamyl transpeptidase(γ-GT),alkaline phosphatase(ALP),bile acid(TBA),serum total cholesterol(TC),triglyceride(TG),low-density lipoprotein(LDL-C)and high-density lipoprotein(HDL-C)in the observation group were significantly lower than those in the control group,and the incidence of complications was significantly lower than that in the control group(P<0.05).Conclusions SMOF can reduce the incidence of complications in VLBW infants,reduce the impact on liver function and blood lipids,which facilitates physical recovery.
