天麻钩藤汤加减对原发性高血压病肝阳上亢型的疗效及血压水平、神经递质节律、中医症状积分的影响分析

An analysis of the effects of modified Tianma Gouteng decoction on blood pressure,neurotransmitter rhythm and TCM symptom score in patients with primary hypertension of hyperactivity of liver Yang

目的:综合研究探析原发性高血压病肝阳上亢型患者使用天麻钩藤汤加减治疗的效果以及对血压水平、神经递质节律、中医症状积分的影响。方法:选取2020年1月―2021年2月被莘县中医医院收治的92例原发性高血压病肝阳上亢型患者作为研究对象,通过随机数字表法分为两组:对照组和观察组,每组46例,对照组使用硝苯地平治疗,观察组在对照组的基础上加用天麻钩藤汤加减治疗,比较两组临床疗效、血压水平、神经递质节律及中医症状积分。结果:治疗前,两组的血压水平、神经递质节律及中医症状积分差异无统计学意义(P> 0.05);在接受不同方案治疗后,与对照组的76.09%(35/46)比较,观察组所获93.48%(43/46),说明观察组临床疗效更高,组间差异有统计学意义(P <0.05);观察组收缩压(Systolic Blood Pressure,SBP)、舒张压(Diastolic Blood Pressure,DBP)、神经肽Y(Neuropeptide Y,NPY)等指标水平与对照组相比更低,甘氨酸(Glycine,Gly)水平与对照组相比更高,组间差异有统计学意义(P<0.05);观察组的中医症状积分下降程度与对照组相比更明显,组间差异有统计学意义(P<0.05)。结论:对原发性高血压病肝阳上亢型患者使用天麻钩藤汤加减治疗可获得较为积极的临床治疗效果,能够有效地降低患者的血压水平,改善其神经递质节律,缓解各项临床症状,是一种较为安全、有效的治疗方式,具有较高的临床应用价值,值得推广。

Objective:To comprehensively study the effects of modified Tianma Gouteng decoction(天麻钩藤汤)on blood pressure,neurotransmitter rhythm and TCM symptom score in patients with primary hypertension of hyperactivity of liver Yang(阳).Methods:A total of 92 patients with primary hypertension of hyperactivity of liver Yang type admitted to Shenxian Hospital of Traditional Chinese Medicine from January 2020 to February 2021 were selected as the research subjects.They were randomly divided into two groups:the control group and the observation group,46 cases in each.The control group was treated with nifedipine.The observation group was treated with modified Tianma Gouteng decoction on the basis of the treatment in the control group.The clinical efficacy,blood pressure level,neurotransmitter rhythm and TCM symptom score were compared between the two groups.Results:Before treatment,there was no significant difference in blood pressure level,neurotransmitter rhythm and TCM symptom score between the two groups(P>0.05).After receiving different treatments,compared with the total efficiency of 76.09%(35/46)in the control group,93.48%(43/46)was obtained in the observation group,indicating that the clinical efficacy in the observation group was higher,and the difference between groups was statistically significant(P<0.05).The levels of SBP,DBP,NPY and other indicators in the observation group were lower than those in the control group,and the levels of Gly in the observation group was higher than the control group.The difference between groups was statistically significant(P<0.05).The decrease of TCM symptom score in the observation group was more significant than that in the control group,and the difference between the groups was statistically significant(P<0.05).Conclusion:Modified Tianma Gouteng decoction can obtain a more positive clinical treatment effect for patients with primary hypertension of liver Yang hyperactivity type,which can effectively reduce the blood pressure level of patients,improve the rhythm of neurotransmitters,and alleviate various clinical symptoms.It is a relatively safe and effective treatment method,which is with high clinical application value,and si worth promotion.
