鄢路洲老师通过长期的临床实践并观察现代人的生活方式,归纳总结出颈源性眩晕发生的病因病机,根据病因病机确定治疗思路,辨证施治,标本兼治,配合《解剖列车》中神经-循环-筋膜系统理论,提出了“挖拍理论”治疗颈源性眩晕的临床思路。从补虚的角度独取足太阳膀胱经穴位(如五脏腧穴以调节五脏之气血阴阳)和督脉穴位(如血会之膈俞穴以补血活血)以振奋阳气,调整全身阴阳,补益气血,恢复全身正气;从泻实的角度运用刃针和火罐治疗局部(长期的病理产物堆积而形成的阿是穴,改善局部微循环)以引邪外出,祛瘀生新,温经通络,不伤正气,两者结合,起效迅速,疗效持久。结合临床医案,充分印证了刃针膈俞穴、五脏腧穴、阿是穴后拔罐放血治疗颈源性眩晕的有效性。最后总结,“挖拍理论”为针灸临床医生提供了一种安全有效、简洁明了的临床治疗思路,如颈椎病引起的疼痛、上肢感觉异常、失眠、耳鸣、皮肤病等,腰椎间盘突出症引起的疼痛、腰部感觉异常、下肢感觉异常等,其他杂病如腱鞘炎、腱鞘囊肿、跟骨骨刺及痛经等,还有前列腺炎、腹胀、肌肉松弛性肥胖、便秘等内科疾病。同时,也为推广针灸临床文化做出了极大的贡献,极大地推动了针灸文化的临床发展。

Through long-term clinical practice and observation of modern people’s lifestyle,Professor YAN Luzhou summarizes the etiology and pathogenesis of cervical vertigo,determines the treatment ideas according to the etiology and pathogenesis,differentiates treatment,treats both the symptoms and the root causes,and puts forward the clinical idea of“digging and padding theory”for the treatment of cervical vertigo in combination with the theory of neural-circulatory-fascial system in Anatomy Train.From the perspective of replenishing deficiency,the foot solar bladder meridian points(such as Wuzang Shu to regulate Qi(气),blood,Yin and Yang(阴阳)of the five internal organs]and Du meridian points(such as Geshu of the blood meeting to replenish blood and activate blood)are taken alone to invigorate Yang Qi,adjust the Yin and Yang of the whole body,replenish Qi and blood,and restore the healthy Qi of the whole body.From the perspective of purging,blade needles and cupping are used to treat local areas[long-term accumulation of pathological products,Ashi(阿是)point to improve local microcirculation]to induce evil spirits to go out,remove blood stasis and generate new blood,warm meridians without harming healthy Qi,the combination of the two shows rapid onset and achieves long-lasting curative effect.Combined with clinical medical cases,it fully confirms the effectiveness of cupping and bloodletting after blade acupuncture at acupoints of Geshu,Wuzang-shu and Ashi in the treatment of cervical vertigo.Finally,in summary,the“digging and padding theory”provides acupuncture clinicians with a safe,effective,concise and clear clinical treatment idea,such as in treating pain caused by cervical spondylosis,upper limb paresthesia,insomnia,tinnitus,skin diseases,pain caused by lumbar disc herniation,lumbar paresthesia,lower limb paresthesia,etc.,other miscellaneous diseases such as tenosynovitis,tendon cyst,calcaneal spur,dysmenorrhea etc.,as well as prostatitis,abdominal distention,muscle relaxation obesity,constipation and other internal medical diseases.At the same time,it has also made great contributions to the promotion of acupuncture clinical culture,and greatly promoted the clinical development of acupuncture culture.
