社区高血压干预模式在高血压患者中的防治效果

目的探讨社区高血压干预模式在高血压患者中的防治效果。方法选取2018年4月至2021年4月赣州市人民医院心血管内科收治的600例高血压患者作为研究对象。通过随机数字表法将患者分为自我管理组(n=200)、家庭管理组(n=200)和社区管理组(n=200)。其中自我管理组采用自我管理干预方法,家庭管理组采用家庭管理干预模式,社区管理组采用社区高血压干预模式。比较三组患者的高血压疾病知晓率、血压控制率、护理满意度、用药依从性。结果三组患者的疾病知晓率比较,差异有统计学意义(P<0.05)。组间两两比较,自我管理组和家庭管理组的疾病知晓率比较,差异无统计学意义(P>0.017);社区管理组的疾病知晓率高于自我管理组和家庭管理组,差异有统计学意义(P<0.017)。三组患者的血压控制率比较,差异有统计学意义(P<0.05)。组间两两比较,自我管理组和家庭管理组的血压控制率比较,差异无统计学意义(P>0.017);社区管理组的血压控制率高于自我管理组和家庭管理组,差异有统计学意义(P<0.017)。三组患者的护理满意度比较,差异有统计学意义(P<0.05)。组间两两比较,自我管理组和家庭管理组的护理满意度比较,差异无统计学意义(P>0.017);社区管理组的护理满意度高于自我管理组和家庭管理组,差异有统计学意义(P<0.017)。三组患者的用药依从性比较,差异有统计学意义(P<0.05)。组间两两比较,自我管理组和家庭管理组的用药依从性比较,差异无统计学意义(P>0.017);社区管理组的用药依从性高于自我管理组和家庭管理组,差异有统计学意义(P<0.017)。结论高血压社区干预模式对于社区内高血压患者的防治效果优于自我管理和家庭干预模式,能够明显提高患者的疾病知晓率、血压控制率和用药依从性,且护理满意度较高。

Objective To explore the effect of community hypertension intervention model in the prevention and treatment of hypertension.Methods A total of 600 patients with hypertension admitted to the Department of Cardiovascular Medicine of Ganzhou People's Hospital from April 2018 to April 2021 were selected as the research objects.The patients were divided into a self-management group(n=200),a family management group(n=200)and a community management group(n=200)by random number table method.The self-management group adopted self-management intervention methods,the family management group adopted family management intervention model,and the community management group adopted community hypertension intervention model.The awareness rate of hypertension,blood pressure control rate,nursing satisfaction and medication compliance were compared among the three groups.Results There was statistically significant difference in disease awareness rate among the three groups(P<0.05).There was no significant difference in the disease awareness rate between the self-management group and the family management group(P>0.017).The disease awareness rate of the community management group was higher than that of the self-management group and the family management group,and the differences were statistically significant(P<0.017).There was statistically significant difference in blood pressure control rate among the three groups(P<0.05).There was no significant difference in blood pressure control rate between the self-management group and the family management group(P>0.017).The blood pressure control rate of the community management group was higher than that of the self-management group and the family management group,and the differences were statistically significant(P<0.017).There was statistically significant difference in nursing satisfaction among the three groups(P<0.05).There was no significant difference in nursing satisfaction between the self-management group and the family management group(P>0.017).The nursing satisfaction of the community management group was higher than that of the self-management group and the family management group,and the differences were statistically significant(P<0.017).There was statistically significant difference in medication compliance among the three groups(P<0.05).There was no significant difference in medication compliance between the self-management group and the family management group(P>0.017).The medication compliance of the community management group was higher than that of the self-management group and the family management group,and the differences were statistically significant(P<0.017).Conclusion The community intervention model of hypertension is better than the self-management and family intervention model in the prevention and treatment of hypertension in the community,which can significantly improve the disease awareness rate,blood pressure control rate and medication compliance of patients,and the nursing satisfaction is higher.
