侧卧位分娩配合无保护会阴接生在基层医院临床应用效果的研究

Study on the clinical application effect of side lying position delivery combined with unprotected perineal delivery in grassroots hospitals

目的探讨侧卧位分娩配合无保护会阴接生在基层医院临床应用的效果。方法选择2022年7月至2023年5月暨南大学附属第一医院潮汕医院的72例孕妇作为研究对象,采用随机数字表法将其分为对照组(n=36)和研究组(n=36)。对照组采用传统分娩配合无保护会阴接生,研究组采用侧卧位分娩配合无保护会阴接生。比较两组产妇分娩后会阴裂伤率、会阴疼痛程度、会阴并发症发生率、产后出血发生率及产后舒适度。结果研究组Ⅰ度会阴损伤率低于对照组,差异有统计学意义(P<0.05);研究组会阴疼痛程度轻于对照组,差异有统计学意义(P<0.05);研究组会阴并发症中脏器脱垂、会阴感染、肛门失禁、会阴异常下降发生率低于对照组,差异有统计学意义(P<0.05);干预后,研究组生理及心理精神评分高于对照组,差异有统计学意义(P<0.05)。结论侧卧位分娩配合无保护会阴接生用于基层医院中,能降低产后会阴裂伤率、会阴并发症发生率,未增加产后出血发生率,有助于减轻会阴疼痛程度,提高产妇产后舒适度,值得推广应用。

Objective To explore the clinical application effect of lateral position delivery combined with unprotected perineal delivery in grassroots hospitals.Methods From July 2022 to May 2023,72 pregnant women were prospectively selected as the subjects.Routine nursing was performed after admission.They were divided into control group(n=36)and experimental group(n=36)by random number table method.The control group used traditional delivery combined with unprotected perineal delivery,while the experimental group used lateral lying delivery combined with unprotected perineal delivery.The rate of perineal laceration,degree of perineal pain,incidence of perineal complications,incidence of postpartum hemorrhage,and postpartum comfort between two groups of postpartum women were compared.Results The incidence ofⅠdegree perineal laceration in the experimental group was lower than that in the control group,with a statistically significant difference(P<0.05).The degree of perineal pain in the experimental group was milder than that in the control group,with statistically significant difference(P<0.05).The incidence of organ prolapse,perineal infection,anal incontinence,and abnormal decrease in perineal complications in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).After intervention,the physiological and psychological scores of the experimental group were higher than those of the control group,with statistically significant differences(P<0.05).Conclusion The use of lateral lying position delivery combined with unprotected perineal delivery in grassroots hospitals can reduce the incidence of postpartum perineal laceration and complications,without increasing the incidence of postpartum bleeding.It helps to reduce perineal pain and improve postpartum comfort,and is worth promoting and applying.
