LncRNA H19和miR-625-5p在消化性溃疡合并幽门螺杆菌感染患儿血清中的表达及意义

Expression and significance of serum LncRNA H19 and miR-625-5p in children with peptic ulcer complicated by Helicobacter pylori infection

摘要 目的分析长链非编码RNA(LncRNA)H19和微小RNA-625-5p(miR-625-5p)在消化性溃疡(PU)合并幽门螺杆菌(HP)感染患儿血清中的表达水平及意义。方法选取2021年2月至2023年2月在泉州市儿童医院治疗的160例PU患儿,其中90例患儿合并HP感染(阳性组),70例未合并HP感染(阴性组),另收集同期160例体检健康儿童为健康人对照组。采用实时荧光定量PCR(qRT-PCR)检测各组LncRNA H19和miR-625-5p的相对表达水平;Pearson法分析PU合并HP感染患儿LncRNA H19和miR-625-5p的相关性;Logistic回归分析LncRNA H19、miR-625-5p与PU患儿HP感染的关联;ROC曲线分析LncRNA H19和miR-625-5p表达对PU患儿HP感染的诊断价值。结果与健康人对照组相比,HP阴性组、阳性组PU患儿血清LncRNA H19水平依次显著升高(P<0.05),miR-625-5p水平依次显著降低(P<0.05);与HP感染Ⅰ型相比,HP感染Ⅱ型PU患儿血清LncRNA H19水平显著降低(P<0.05),miR-625-5p水平显著升高(P<0.05);LncRNA H19与miR-625-5p间存在结合位点,且PU合并HP感染患儿血清LncRNA H19与miR-625-5p水平呈显著负相关(r=-0.536,P<0.05)。Logistic回归分析发现,LncRNA H19是影响PU患儿HP感染的危险因素(P<0.05),miR-625-5p是PU患儿HP感染的保护因素(P<0.05);LncRNA H19、miR-625-5p诊断PU患儿HP感染的ROC曲线下面积(AUC ROC)分别为0.870和0.895,二者联合诊断的AUC ROC为0.935,优于其各自单独诊断(Z二者联合-LncRNA H19=1.991,P=0.023;Z二者联合-miR-625-5P=1.707,P=0.044)。结论PU合并HP感染患儿血清LncRNA H19水平升高,miR-625-5p水平降低,二者对PU合并HP感染具有一定的诊断价值。 Objective To analyze the expression and significance of serum long non-coding RNA(LncRNA)H19 and micro RNA-625-5p(miR-625-5p)in children with peptic ulcer(PU)complicated by Helicobacter pylori(HP)infection.Methods A total of 160 children with PU who were treated in our hospital from February 2021 to February 2023 were selected,including 90 patients with HP infection(positive group)and 70 patients without HP infection(negative group),and 160 healthy children in the same period as the control group.The relative expression levels of LncRNA H19 and miR-625-5p were detected by real-time fluorescence quantitative PCR(qRT-PCR).The Pearson method was applied to analyze the correlation between the expression levels of LncRNA H19 and miR-625-5p in children with PU and HP infection.The association of LncRNA H19h and miR-625-5p with HP infection in children with PU was analyzed by the logistic regression,and the value of the expression of LncRNA H19 and miR-625-5p in the diagnosis of HP infection in children with PU was analyzed by the receiver operating characteristic(ROC)curve analysis.Results Compared with the control group,the levels of serum LncRNA H19 in the HP negative group and positive group were obviously increased in turn(P<0.05),and the levels of serum miR-625-5p were obviously decreased in turn(P<0.05).Compared with HP infection typeⅠ,the levels of serum LncRNA H19 in children with HP infection typeⅡwere obviously reduced(P<0.05),and the levels of serum miR-625-5p were obviously increased(P<0.05).There was a binding site between LncRNA H19 and miR-625-5p,and there was an obvious negative correlation between serum LncRNA H19 and miR-625-5p levels in children with PU complicated by HP infection(r=-0.536,P<0.05).Logistic regression analysis showed that LncRNA H19 was a risk factor for HP infection in children with PU(P<0.05),and that miR-625-5p was a protective factor affecting HP infection(P<0.05).The areas under the ROC curve(AUC ROC)of serum LncRNA H19 and miR-625-5p in diagnosing HP infection of children with PU were 0.870 and 0.895,respectively,and the AUC ROC of their combination was 0.935,which was better than their respective diagnosis alone(Z combination-LncRNA H19=1.991,P=0.023;Z combination-miR-625-5P=1.707,P=0.044).Conclusion The levels of serum LncRNA H19 in children with PU complicated by HP infection increase,while the levels of serum miR-625-5p decrease,which may have certain diagnostic value for PU complicated with HP infection.
