AMI介入术后尿微量白蛋白与PCT表达水平及与心肌微循环障碍的关系

摘要 目的探讨急性心肌梗死(AMI)介入术后尿微量白蛋白(UmAlb)与降钙素原(PCT)表达水平及与心肌微循环障碍的关系。方法选取2020年4月至2023年3月我院收治的100例AMI患者,根据心肌声学造影结果分为微循环障碍组和正常组。检测患者PCT、UmAlb水平,分析二者与介入术后心肌微循环障碍的关系。结果Logistic多因素回归分析显示,高血压、术前TIMI血流<3级、PCT、24h-UmAlb为AMI介入术后微循环障碍的独立危险因素(P<0.05)。PCT及24h-UmAlb联合检测预测介入术后心肌微循环障碍的AUC高于PCT、24h-UmAlb单独检测(P<0.05)。结论PCT及UmAlb为AMI患者介入术后心肌微循环障碍的危险因素,二者联合检测有助于早期预测心肌微循环障碍的发生. Objective To explore the relationship between urinary microalbumin(UmAlb)and procalcitonin(PCT)expression levels and myocardial microcirculation disorder after acute myocardial infarction(AMI)intervention.Methods 100 AMI patients admitted to our hospital from April 2020 to March 2023 were selected and divided into microcirculation disorder group and normal group according to myocardial contrast echocardiography results.The PCT and UmA1b levels were detected to analyze the relationship between PCT and UmAlb levels and myocardial microcirculation disorder after intervention.Results Multivariate Logistic regression analysis showed that hypertension,preoperative TIMI blood flow<grade 3,PCT and 24h-UmA1b were independent risk factors for microcirculation disorder after AMI intervention(P<0.05).The AUC of PCT combined with 24h-UmAlb detection was higher than that of single detection of PCT and 24h-UmA1b(P<0.05).Conclusions PCT and UmAlb are risk factors for myocardial microcirculation disorder in AMI patients after intervention,and the combined detection is helpful to early predict the occurrence of myocardial microcirculation disorder.
