多囊卵巢综合征患者血清ANGPTL8、CTRP12水平及其与胰岛素抵抗、性激素指标的相关性分析 被引量:1

Analysis of serum levels of ANGPTL8 and CTRP12 in patients with polycystic ovary syndrome and their correlation with insulin resistance and sex hormone indicators

摘要 目的探究多囊卵巢综合征(PCOS)患者血清血管生成素样蛋白8(ANGPTL8)、补体C1q/肿瘤坏死因子相关蛋白12(CTRP12)水平及其与性激素指标、胰岛素抵抗的相关性分析。方法选取2021年1月至2023年1月在该院就诊的PCOS患者106例,按照体质量指数(BMI),将PCOS患者分为肥胖组(49例)和非肥胖组(57例);另选取60例同期体检健康女性为对照组。比较受试者一般资料及血清CTRP12、ANGPTL8水平。用Pearson法分析CTRP12、ANGPTL8水平与PCOS患者性激素指标及胰岛素抵抗的相关性。采用多元线性回归分析PCOS患者血清ANGPTL8、CTRP12水平的影响因素。结果与对照组相比,肥胖组、非肥胖组舒张压(DBP)、空腹血糖(FPG)、收缩压(SBP)、空腹胰岛素(FINS)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、餐后2 h血糖(2hPG)、胰岛素抵抗指数(HOMA-IR)、黄体生成素(LH)、总胆固醇(TC)、促卵泡素(FSH)、睾酮(T)、雌二醇(E2)、ANGPTL8水平较高,差异有统计学意义(P<0.05),血清CTRP12、高密度脂蛋白胆固醇(HDL-C)较低,差异有统计学意义(P<0.05),与非肥胖组相比,肥胖组BMI、腰臀比(WHR)、SBP、DBP、FPG、2hPG、FINS、TC、HOMA-IR、LDL-C、TG、ANGPTL8水平较高,差异有统计学意义(P<0.05),血清CTRP12、HDL-C较低,差异有统计学意义(P<0.05)。Pearson分析显示CTRP12与HDL-C呈正相关(P<0.05),与FPG、HOMA-IR、FINS、LDL-C、TC、TG呈负相关(P<0.05);ANGPTL8与FPG、FINS、TC、LDL-C、TG、HOMA-IR呈正相关(P<0.05),与HDL-C呈负相关(P<0.05)。多元线性回归分析显示,HDL-C降低、FPG升高、FINS升高、TC升高、TG升高、HOMA-IR升高、LDL-C升高是ANGPTL8、CTRP12水平的影响因素(P<0.05)。结论PCOS患者血清CTRP12水平下降,ANGPTL8水平上升,ANGPTL8、CTRP12与胰岛素抵抗及TC、TG、LDL-C、HDL-C相关,与LH、FSH、E2、T无相关性。 Objective To investigate the levels of serum angiopoietin-like protein 8(ANGPTL8),complement C1q/tumor necrosis factor-related protein 12(CTRP12)in patients with polycystic ovary syndrome(PCOS)and their correlation with sex hormone levels and insulin resistance.Methods One hundred and six PCOS patients who visited the hospital from January 2021 to January 2023 were selected and divided into an obese group(49 cases)and a non obese group(57 cases)based on their body mass index(BMI);Another 60 healthy women who underwent physical examination during the same period were selected as the control group.Compare the general information of the subjects and the serum levels of CTRP12 and ANGPTL8.The Pearson method was used to analyze the correlation between the levels of CTRP12 and ANGPTL8 with sexual hormone indicators and insulin resistance in patients with PCOS.Multiple linear regression was used to analyze the influencing factors of serum ANGPTL8 and CTRP12 levels in patients with PCOS.Results Compared with the control group,the levels of diastolic blood pressure(DBP),asting blood glucose(FPG),systolic blood pressure(SBP),fasting insulin(FINS),triacylglycerol(TG),low-density lipoprotein cholesterol(LDL-C),2 h postprandial blood glucose(2hPG),homeostasis model assessment of insulin resistance(HOMA-IR),luteinizing hormone(LH),total cholesterol(TC),follicle-stimulating hormone(FSH),testosterone(T),estradiol(E2),and ANGPTL8 in the obese and non-obese groups were higher,the difference was statistically significant(P<0.05),serum CTRP12 and high-density lipoprotein cholesterol(HDL-C)were lower,the difference was statistically significant(P<0.05),compared with the non-obese group,the obese group had higher levels of BMI,waist-hip ratio(WHR),SBP,DBP,FPG,2hPG,FINS,TC,HOMA-IR,LDL-C,TG,ANGPTL8,the difference was statistically significant(P<0.05),and lower serum CTRP12,HDL-C(P<0.05).Pearson analysis showed that CTRP12 was positively correlated with HDL-C(P<0.05),and negatively correlated with FPG,HOMA-IR,FINS,LDL-C,TC and TG(P<0.05);ANGPTL8 was positively correlated with FPG,FINS,TC,LDL-C,TG,HOMA-IR,the difference was statistically significant(P<0.05),and negatively correlated with HDL-C(P<0.05).Multiple linear regression analysis showed that decreased HDL-C,increased FPG,increased FINS,increased TC,increased TG,increased HOMA-IR and increased LDL-C were the influencing factors of ANGPTL8 and CTRP12 levels(P<0.05).Conclusion The level of CTRP12 in PCOS patients decreases,while the level of serum ANGPTL8 increases.ANGPTL8 and CTRP12 are related to glucose and lipid metabolism and insulin resistance,but have no correlation with LH,FSH,E2,and T.
