容量管理模式对心力衰竭患者自我护理行为及心功能的影响

目的观察分析心衰容量管理模式对心衰患者自我护理行为及心功能的改善作用。方法收集2019年2月—2022年2月医院心内科收治的80例心衰患者,在组间基线特征均衡可比的原则上,分为对照组和观察组,各40例,对照组予以常规护理,观察组予以心衰容量管理模式,3个月后对两组患者的自我护理行为、心功能、心理痛苦程度、生存质量进行比较。结果护理干预后,观察组患者的欧洲心力衰竭自我护理量表(EHFScBS-9)评分低于对照组,而观察组的心力衰竭自我护理指数(SCHFI)评分高于对照组,差异均有统计学意义(P<0.05)。观察组患者的左心室射血分数(LVEF)与心输出量(CO)均高于对照组,而左室舒张末期内径(LVEDD)与左室收缩末期内径(LVESD)短于对照组,并且观察组患者的心理痛苦温度计评分低于对照组;观察组护理后明尼苏达心力衰竭生存质量量表(MLHFQ)评分低于对照组,组间差异均有统计学意义(P<0.05)。结论心衰容量管理模式在心衰患者中的应用可取得明显效果,不仅能增强患者的自我护理行为,改善心功能,缓解心理痛苦程度,而且还能促进其生存质量进一步提高。

Objective To observe and analyse the effect of heart failure volume management model on the improvement of self-care behaviours and cardiac function in heart failure patients.Methods 80 cases of heart failure patients admitted to the Department of Cardiology of our hospital from February 2019 to February 2022 were collected,and on the principle of balanced and comparable baseline characteristics between the groups,they were divided into a control group and an observation group,each with 40 cases,the control group was subjected to routine care,and the observation group was subjected to the heart failure volume management mode,and the self-care behaviours,cardiac function,and the degree of psychological pain of the two groups were compared after three months,Survival quality was compared after 3 months.Results After the nursing intervention,the European Heart Failure Self-Care Scale(EHFScBS-9)scores of the patients in the observation group were lower than those of the control group,while the scores of the Heart Failure Self-Care Index(SCHFI)in the observation group were higher than those of the control group,and the differences were all statistically significant(P<0.05).The left ventricular ejection fraction(LVEF)and cardiac output(CO)of the patients in the observation group were higher than those in the control group,while the left ventricular end-diastolic internal diameter(LVEDD)and left ventricular end-systolic internal diameter(LVESD)were shorter than those in the control group,and the patients in the observation group had lower scores on the psychological distress thermometer than those in the control group;and the scores on the post-care Minnesota Heart Failure Quality of Survival Quotient(MLHFQ)were lower than those in the control group.The differences between the groups were all statistically significant(P<0.05).Conclusion The application of heart failure volume management model in heart failure patients can achieve significant results,not only can enhance the patients'self-care behaviours,improve cardiac function,alleviate the degree of psychological pain,but also promote the further improvement of their quality of life.
