浙江省大学生MSM安全套使用效能评价及其相关影响因素分析

Evaluation and related factors of the condom use efficacy among MSM college students in Zhejiang Province

目的了解浙江省大学生MSM安全套使用效能现况,探索相关影响因素,为干预措施制定提供理论依据。方法采用横断面调查方法,于2022年3-7月,在浙江省杭州、宁波、金华、嘉兴这四个MSM较集中的地区选取年龄在18周岁及以上大学生MSM作为研究对象,采用方便样本,利用自行设计的电子问卷采用面对面和自填相结合的方式开展调查,收集基本特征、社会文化、危险行为和干预检测等信息。采用SPSS软件进行χ2检验和多因素Logistic回归分析大学生MSM安全套使用效能相关因素。结果共调查671名大学生MSM,安全套使用效能高者占75.3%(505/671)。多因素Logistic回归分析结果显示,大学生MSM安全套使用效能评价高的保护因素包括与父母关系好(a OR=2.142,95%CI:1.435~3.198)、倾诉对象数量≥1个(a OR=1.863,95%CI:1.128~3.079)、观看网络色情视频频次1~2次/月(a OR=1.935,95%CI:1.122~3.336)、观看网络色情视频频次≥3次/月(a OR=2.276,95%CI:1.432~3.619)、接受安全套同伴教育(a OR=2.113,95%CI:1.385~3.225)、既往进行HIV自我检测(a OR=1.567,95%CI:1.020~2.405),危险因素包括心理性别为女性(a OR=0.415,95%CI:0.192~0.897)、心理性别为双性/不确定(a OR=0.606,95%CI:0.379~0.970)、在MSM圈内不受欢迎(a OR=0.515,95%CI:0.325~0.815)、曾服用暴露后预防药物(a OR=0.516,95%CI:0.281~0.950)。结论浙江省大学生MSM安全套使用效能较高,建议加强大学生MSM心理支持,开展安全套同伴教育,提高检测服务可及性,以提高大学生MSM的安全套使用效能。

Objective To identify the status of condom use efficacy and explore relevant influencing factors among men who have sex with men(MSM)college students in Zhejiang Province,so as to provide theoretical basis for intervention formulation.Methods From March to July 2022,a cross-sectional survey was conducted in four cities with a high concentration of MSM(Hangzhou,Ningbo,Jinhua,and Jiaxing)in Zhejiang Province.By using convenient samples,college students aged 18 and above who have had sex with men as the research subjects were selected as the research subjects.The survey was conducted using self-designed electronic questionnaires,which used a combination of face-to-face and self-administered methods to collect information on basic characteristics,social culture,risky behaviors,intervention testing and other topics.Chi-square test and multivariate logistic regression were used to analyze the factors associated with condom use among MSM college students by SPSS software.Results A total of 671 MSM university students were surveyed,with 75.3%(505/671)having high condom use efficacy.The results of the multifactorial logistic regression analysis showed that,the protective factors related to condom use efficacy among MSM college students included positive relationships with parents(aOR=2.142,95%CI:1.435-3.198),number of confidants≥1(aOR=1.863,95%CI:1.128-3.079),frequency of watching online pornographic videos 1-2 times/monthly(aOR=1.935,95%CI:1.122-3.336),frequency of watching online pornographic videos≥3 times/monthly(aOR=2.276,95%CI:1.432-3.619),peer education about condom use(aOR=2.113,95%CI:1.385-3.225),previous HIV self-test(aOR=1.567,95%CI:1.020-2.405);whereas risky factors included psychological gender as female(aOR=0.415,95%CI:0.192-0.897),psychological gender as intersex/uncertain(aOR=0.606,95%CI:0.379-0.970),unpopularity among MSM(aOR=0.515,95%CI:0.325-0.815)and post-exposure prophylaxis(aOR=0.516,95%CI:0.281-0.950).Conclusions The condom use efficacy of MSM college students in Zhejiang Province is high.It is recommended to strengthen psychological support for MSM college students,carry out condom peer education,and improve the accessibility of testing services,in order to improve the condom use efficacy among MSM college students.
