摘要
目的:探讨沙库巴曲缬沙坦钠片联合替奈普酶治疗急性脑梗死(ACI)患者对其神经功能缺损、血管生成因子水平的影响。方法:选取2021-01-2021-12本院收治的128例ACI患者为研究对象,并随机分为对照组(64例)和观察组(64例),对照组在常规治疗基础上给予替奈普酶治疗,观察组在对照组基础上增加沙库巴曲缬沙坦钠片治疗。比较两组临床疗效及治疗后NIHSS、BI指数、血管生成相关因子[血管内皮生长因子(VEGF)、血管内皮生长因子受体2(VEGFR2)、内皮素1(ET-1)、一氧化氮(NO)]、神经损伤指标[同型半胱氨酸(Hcy)、神经生长因子(NGF)、神经元特异性烯醇化酶(NSE)、脑源性神经营养因子(BDNF)]、醛固酮(ALD)、肾素(PRA)、血管紧张素(AngⅡ)水平。结果:治疗后,观察组总有效率高于对照组(92.19%vs 79.69%,P<0.05);两组NIHSS评分、Hcy、NSE、ALD、PRA、AngⅡ、ET-1水平均下降,而BI指数、VEGF、VEGFR2、NGF、BDNF、NO水平均上升,且观察组较对照组上升/下降更明显(P<0.05)。治疗期间,观察组和对照组不良反应事件发生率比较无统计学差异(9.38%vs 14.06%,P>0.05)。结论:沙库巴曲缬沙坦钠片联合替奈普酶治疗可显著改善急性脑梗死患者神经功能、血管功能,并可调控肾素-血管紧张素-醛固酮系统,临床效果显著。
Objective:To investigate the effects of sacubactril and valsartan sodium tablets combined with teneplase on nerve function deficit and angiogenesis factor levels in patients with acute cerebral infarction.Method:128 patients with acute cerebral infarction admitted to our hospital from January 2021 to December 2021 were selected as research objects,and were randomly divided into control group(64 cases)and observation group(64 cases).The control group was treated with teneplase on the basis of conventional treatment,and the observation group was treated with sacubactril valsartan sodium tablets on the basis of control group.Clinical efficacy,NIHSS,BI index,angiogenesis related factors[vascular endothelial growth factor(VEGF),vascular endothelial growth factor receptor 2(VEGFR2),endothelin 1(ET-1),nitric oxide(NO)]and nerve injury indexes[homocysteine(Hcy),nerve growth factor(NGF),neuron specific enolase(NSE),brain-derived neurotrophic factor(BDNF)],aldosterone(ALD),renin(PRA),angiotensin(AngⅡ)levels were compared between the two groups after treatment.Results:After treatment,the total effective rate of observation group was higher than that of control group(92.19%vs 79.69%,P<0.05).The NIHSS score,Hcy,NSE,ALD,PRA,AngⅡ,ET-1 levels were decreased,while BI index,VEGF,VEGFR2,NGF,BDNF and NO levels were increased in both groups,and the increase/decrease in the observation group was more obvious than that in the control group(P<0.05).During treatment,there was no significant difference in the incidence of adverse events between the observation group and the control group(9.38%vs 14.06%,P>0.05).Conclusion:The combination of sacubactril valsartan sodium tablets and teneplase can significantly improve nerve function and vascular function in patients with acute cerebral infarction,and can regulate renin-angiotensin-aldosterone system,with significant clinical effects.
