加味生肌玉红膏促进湿热证肛瘘术后创面愈合的实验研究

Experimental study on the effect of modified Shengji Yuhong ointment on wound healing after anal fistula operation with damp heat syndrome

目的 研究加味生肌玉红膏对湿热证肛瘘大鼠创面的抗炎和促愈作用。方法 建立湿热证大鼠肛瘘创面模型。将大鼠随机分为空白组、凡士林组、康复新组、加味生肌玉红膏组、PDTC+凡士林组、PDTC+康复新组、PDTC+加味生肌玉红膏组。计算创面愈合率,ELISA法检测创面组织TNF-α表达,免疫组化和Western blot法检测创面组织AQP3表达。结果 (1)干预中后期,PDTC+加味生肌玉红膏组创面愈合率最高。(2)初期各组TNF-α表达均较高,但PDTC+加味生肌玉红膏组在各阶段TNF-α表达均低于其他组。(3)Western blot和免疫组化结果显示初期各组AQP3表达均较高。与其他组相比,PDTC+加味生肌玉红膏组初期AQP3表达偏低,中期AQP3表达较高,后期AQP3表达明显降低。结论 (1)NF-κB/AQP信号通路调控失常导致TNF-α和AQP3升高与湿热证的形成密切相关。(2)加味生肌玉红膏可调控NF-κB/AQP信号通路,调节TNF-α和AQP3表达,减轻湿热证肛瘘创面的炎症反应,促进创面愈合。

Objective To study the anti-inflammatory and healing effects of modified Shengji Yuhong ointment on the wound of anal fistula rats with damp-heat syndrome.Methods The wound model of anal fistula in rats with damp-heat syndrome was es⁃tablished.The rats were divided into blank group,vaseline group,Kangfuxin group,modified Shengji Yuhong ointment group,PDTC+vaseline group,PDTC+Kangfuxin group and PDTC+modified Shengji Yuhong ointment group.The wound healing rate was calculated,the expression of TNF-αin wound tissue was detected by ELISA,and the expression of AQP3 in wound tissue was detected by immunohistochemistry and Western blot.Results In the middle and late stage of intervention,the wound healing rate of PDTC+modified Shengji Yuhong ointment group was the highest.The expression of TNF-αin each group was higher at the initial stage,but the expression of TNF-αin PDTC+modified Shengji Yuhong ointment group was lower at each stage.The results of Western blot and immunohistochemistry showed that AQP3 expression was higher in each group in the early stage.In PDTC+modified Shengji Yuhong Ointment group,the expression of AQP3 was lower in the initial stage,higher in the middle stage,and significantly lower in the later stage.Conclusion①The increase of TNF-αand AQP3 is related to damp-heat.②Modified Shengji Yuhong ointment can regulate the expression of TNF-αand AQP3,reduce the inflammatory reaction of anal fis⁃tula wound with damp-heat syndrome,and promote wound healing.
