金威尔教授治疗糖尿病性视网膜病变处方用药分析

Analysis of Professor Jin Weier's Medication for Diabetic Retinopathy

目的:通过数据挖掘对福建省名中医金威尔教授治疗糖尿病性视网膜病变(DR)的处方进行用药规律分析,为临床治疗DR提供用药依据。方法:收集2012年12月1日-2022年9月30日金威尔教授在福建中医药大学附属人民医院门诊治疗糖尿病性视网膜病变的处方,基于中医传承辅助平台(V2.5)建立DR处方数据库,通过数据挖掘技术分析所含中药的使用频次、药物属性、用药模式及组方规律。结果:共纳入处方327首,涉及中药170味。其中使用频次最高的中药为黄芪、苍术、玄参、茯苓、生地黄等;药性以寒性、温性、平性居多;药味以甘味、苦味、辛味为主;归经多属肝、脾、肾经。常见药对为“玄参,苍术”“黄芪,苍术”“玄参,黄芪”“玄参,黄芪,苍术”“生地黄,黄芪”,关联规则分析得出药物组合分别为:“黄芪,玄参→苍术”“玄参→苍术”“生地黄,黄芪→玄参”“丹参,苍术→玄参”“生地黄,茯苓→枸杞子”。结论:金威尔教授治疗DR用药常选甘温、苦寒、辛散之品,以补肝益肾滋阴、活血祛瘀明目、健脾益气祛湿为主,常用方剂为益景汤、杞菊地黄汤、参苓白术散等。

Objective:Through the data mining to summarize Professor Jin Weier's experience in the treatment of diabetic retinopathy(DR),so as to provide medical basis for clinical treatment of DR.Methods:Collect prescriptions of diabetic retinopathy treated by Professor Jin Wei-er in the outpatient department of the People's Hospital affiliated to Fujian University of Traditional Chinese Medicine(TCM)from December 2012 to September 2022,and establish the relevant prescription database on the TCM inheritance assistance platform(V2.5).Then through the data mining to explore the use frequency,drug attribute,drug use mode and prescription rule.Results:170 Chinese herbal medicine(CHM)were involved among 327 selected prescriptions.The highest frequency of the CHM were Astragalus membranaceus,Rhizoma atractylodis,Radix scrophulariae,Poria cocos,Rawradixrehmanniae.The main Qi of CHM were cold,warm and neutral,of which the most flavors were sweet,bitter,pungent.The meridians belonged to the meridians of liver,spleen,kidney.The Common CHM combination were that“figwort root,rhizoma atractylodis”“radix astragali,rhizoma atractylodis”“radix scrophulariae,radix astragali”“radix scrophulariae,radix astragali,rhizoma atractylodis”“raw radix rehmanniae,radix astragali”.The medicine combination after analysizing by association rules were“radix astragali,radix scrophulariae→rhizoma atractylodis”“Figwort root→rhizoma atractylodis”“raw radix rehmanniae,radix astragali→radix scrophulariae”“salvia miltiorrhiza,rhizoma atractylodis→radix scrophulariae”“raw radix rehmanniae,poria cocos→fructus lycii”.Conclusion:Professor Jin Wei-er's clinical treatment of DR is based on“nourishing liver,kidney and Yin,promoting blood circulation and removing blood stasis,reinforcing Qi strength spleen and clearing damp”.The mainly used CHM were the Ganwen,Kuhan,Xinsan CHM,and the mainly used prescriptions were Yijing Decoction,Qijudihuang Decoction,Shenlingbaizhu Decoction.
