Nd∶YAG激光辅助龈下刮治对Ⅲ期牙周炎残留牙周袋的疗效影响

Clinical Efficacy:Management of Residual Pocket of StageⅢPeriodontits with Nd∶YAG Laser-Assisted Subgingival Scaling

目的探讨Nd∶YAG激光辅助龈下刮治对Ⅲ期牙周炎患者牙周基础治疗后残留牙周袋临床疗效的影响。方法选择2022年4月至2022年8月,Ⅲ期牙周炎患者14例,以牙周基础治疗后4~6周再评估时探诊深度(probing depth,PD)≥5 mm,伴探诊出血(bleeding on probing,BOP),或PD≥6 mm的残存牙周袋为研究对象,采用自身对照研究,口内一侧为实验组,行Nd∶YAG激光联合龈下刮治根面平整;另一侧为对照组,仅行龈下刮治根面平整。比较两组在基线、治疗后6周治疗后3个月PD、附着丧失(attachment loss,AL)和出血指数(bleeding index,BI)的改变。结果治疗后6周和治疗后3个月,两组PD、AL及BI水平较基线明显降低(P<0.05),对于单根牙以及5 mm≤PD≤6 mm的位点,实验组治疗后6周和治疗后3个月PD、BI值均低于对照组(P<0.05)。结论Nd∶YAG激光辅助龈下刮治可提升Ⅲ期牙周炎中单根牙和5 mm≤PD≤6 mm位点的残留牙周袋的疗效。

Objective To explore the clinical effects of Nd∶YAG laser-assisted subgingival scaling of stageⅢperiodontits patients on residual pockets.Methods Totally 14 patients with stageⅢperiodontits were recruited.After 4-6 weeks’initial treatment,the residual pockets with a probing depth(PD)≥5 mm and bleeding on probing,or with a PD≥6 mm were randomly allocated in a split-mouth design to either compose a Nd∶YAG laser therapy group(test group)or a subgingival scaling and root planing group(control group).The clinical indicators of the two groups such as PD,attachment loss(AL)and bleeding index(BI)were measured respectively at baseline and 6 weeks and 3 months after the treatment and compared.Results Great improvement were found in the clinical parameters of both groups after the treatment(P<0.05).For sites in a singlerooted teeth and sites of certain depths(5 mm≤PD≤6 mm),the PD and BI in the test group was statistically lower than that in the con⁃trol group 6 w and 3 m after the treatment(P<0.05).Conclusions Nd∶YAG laser-assisted subgingival scaling for patients with stageⅢperiodontits on residual pockets is more effective than subgingival scaling alone for single-rooted teeth and sites of certain depths(5 mm≤PD≤6 mm).
