针刀联合芪芍通痹胶囊治疗膝骨关节炎(寒湿痹阻证)的临床观察

Clinical Efficacy of Acupotomy Combined with Qishao Tongbi Capsules in the Treatment of Knee Osteoarthritis with Cold-dampness Blockade Liu

目的观察针刀联合芪芍通痹胶囊治疗膝骨关节炎(KOA)(寒湿痹阻证)的临床疗效。方法按照随机数字表法将120例患者分为针刀组、中成药组及联合组各40例。各组疗程均为4周。观察各组治疗前后视觉模拟量表(VAS)评分、Lysholm、WOMAC评分及血清红细胞沉降率(ESR)、C反应蛋白(CRP)、白细胞介素-1β(IL-1β)水平。结果各组治疗4周后VAS、Lysholm、WOMAC评分水平均优于治疗前,且联合组治疗后各项评分显著优于针刀组及中成药组(P<0.05)。各组治疗4周后血清ESR、CRP、IL-1β水平较治疗前均明显降低,且联合组治疗后3项水平显著低于针刀组及中成药组(P<0.05)。联合组总有效率为92.50%,高于针刀组的82.50%和中成药组的77.50%(P<0.05)。结论针刀联合芪芍通痹胶囊治疗KOA(寒湿痹阻证)能有效改善膝关节疼痛及活动功能,调节血清ESR、CRP、IL-1β水平,对KOA治疗具有较好的临床疗效。

Objective:To explore the clinical effect of acupotomy combined with Qishao Tongbi Capsules in the treatment of cold-dampness blockade knee osteoarthritis.Methods:A total of 120 KOA patients meeting the inclusion criteria were selected as research objects,and were divided into acupotomy group,Chinese patent medicine group and combined treatment group with 40 cases each according to random number method.The treatment course of the three groups was 4 weeks.VAS,Lysholm and WOMAC scores and serum ESR,CRP and IL-1βscores were observed before and after treatment in the three groups.Results:VAS,Lysholm and WOMAC scores of the three groups were significantly improved after 4 weeks of treatment than before treatment,and the three scores of the combined group were significantly superior to those of the acupotomy group and the Chinese patent medicine group after treatment,the difference was statistically significant(P<0.05).After 4 weeks of treatment,serum ESR,CRP and IL-1βlevels in the three groups were significantly lower than those before treatment,and the three indicators in the combined group were significantly lower than those in the acupotomy group and the Chinese patent medicine group after treatment,with statistical significance(P<0.05).After 4 weeks of treatment,the total effective rate of the combined group was 92.50%,the acupotomy group and the Chinese patent medicine group were 82.50%and 77.50%,respectively.The combined group had better efficacy than the acupotomy group and the Chinese patent medicine group(P<0.05).Conclusion:Acupotomy combined with Qishao Tongbi Capsules in the treatment of cold-dampness blockade knee osteoarthritis can effectively improve knee pain and activity function,regulate serum ESR,CRP,IL-1βlevels,and have a good clinical effect on knee joint treatment.
