彩色多普勒超声下定位微创旋切在触诊阴性乳腺结节治疗中的有效性及美观性

Effectiveness and aesthetics of minimally invasive circumcision under color Doppler ultrasound in the treatment of palpable negative breast nodules

目的研究彩色多普勒超声下定位微创旋切在触诊阴性乳腺结节治疗中的有效性及美观性。方法选取2021年4月至2023年1月南城博瑷医院收治的60例触诊阴性乳腺结节患者为研究对象,根据手术方式分为观察组(30例)和对照组(30例),对照组实施常规乳腺结节切除术治疗,观察组实施彩色多普勒超声下定位微创旋切术治疗。比较2组围术期指标、术后疼痛程度、切口瘢痕情况、乳房美观度和术后并发症。结果2组术后病理结果比较差异无统计学意义(P>0.05);观察组手术时间、术中出血量、瘢痕长度、切口愈合时间均短于对照组(t=20.690、16.161、38.176、14.388,P<0.05);观察组术后1 d、2 d、3 d疼痛视觉模拟评分(VAS)、术后3个月温哥华瘢痕量表(VSS)各维度(柔软度、色泽、厚度、血管分布)评分及总分、术后并发症发生率均低于对照组(t=3.188、5.226、5.340,t=2.249、3.092、3.006、2.673、5.036;χ^(2)=4.320,均P<0.05);观察组乳房美观度高于对照组(χ^(2)=6.405,P<0.05)。结论相较于常规乳腺结节切除术,彩色多普勒超声下定位微创旋切在触诊阴性乳腺结节患者的应用效果显著,可有效减轻对患者的创伤,降低术后疼痛程度,促进瘢痕恢复,且术后并发症少。

Objective To study the effectiveness and aesthetics of minimally invasive circumcision under color Doppler ultrasound in the treatment of palpable negative breast nodules.Methods Sixty patients with palpation negative breast nodules admitted to Nancheng Boai Hospital from April 2021 to January 2023 were selected as the study subjects.They were randomly divided into an observation group(n=30)and a reference group(n=30)based on surgical procedures with the control group received routine breast nodulectomy treatment,while the observation group received minimally invasive circumcision under color ultrasound localization.The perioperative indicators,postoperative pain level,incision scars,breast aesthetics,and postoperative complications were compared between the two groups.Results There was no statistically significant difference in postoperative pathological results between the two groups(P>0.05).The observation group’s surgical time,intraoperative bleeding,scar length,and incision healing time were shorter than those of the control group(t=20.690,16.161,38.176,14.388,P<0.05).The observation group had lower pain visual analogue scale(VAS)scores on the 1st,2nd,and 3rd postoperative days,as well as the Vancouver Scar Scale(VSS)scores and total scores on all dimensions(softness,color,thickness,vascular distribution)at 3 months after surgery,and the incidence of postoperative complications compared to the control group(t=3.188,5.226,5.340,t=2.249,3.092,3.006,2.673,5.036;χ^(2)=4.320,all P<0.05).The aesthetics of the breasts in the observation group was higher than that of the control group(χ^(2)=6.405,P<0.05).Conclusion Compared with conventional breast nodule resection,color ultrasound guided minimally invasive circumcision has a significant effect in patients with negative palpation of breast nodules.It is effective in reducing trauma to patients,reducing the degree of postoperative pain,and promoting scar recovery,with fewer postoperative complications.
