摘要
目的:分析外周血B淋巴细胞(CD3^(-)CD19^(+))、白细胞介素(IL)-6对系统性红斑狼疮(SLE)发生贫血的影响,以探讨降低SLE患者贫血风险的潜在靶点。方法:收集2020年3月—2022年2月接受诊治的70例SLE合并贫血患者的病例资料作为发生组;另收集同期收治的66例SLE未合并贫血的患者病例资料,作为未发生组。查阅病例资料中研究所需的检测指标,入院检查项目包括T淋巴细胞(CD4^(+)),CD3^(-)CD19^(+),血清IL-6及血常规等SLE相关指标,根据统计结果构建logistic回归模型,分析CD3^(-)CD19^(+),血清IL-6对SLE患者发生贫血的影响,使用Pearson相关性分析SLE患者CD3^(-)CD19^(+)与血清IL-6之间的关系。结果:发生组疾病活动度指数(SLEDAI)评分、CD4^(+)、CD3^(-)CD19^(+)计数、血清IL-6表达量均高于未发生组,肾脏病变占比多于未发生组,差异有统计学意义(P<0.05);2组其他资料比较,差异无统计学意义(P>0.05)。构建logistic回归模型,SLEDAI高评分、CD3^(-)CD19^(+)、IL-6高表达、肾脏病变均能够增加SLE患者贫血发生风险(OR>1,P<0.05);使用Pearson相关性检验发现,SLE患者外周血CD3^(-)CD19^(+)与IL-6呈正相关(r>0,P<0.05)。结论:B细胞、IL-6在SLE合并贫血患者中高度表达;B细胞、IL-6高表达、疾病活动及肾脏病变均能够增加SLE患者贫血发生风险。
Objective To analyze the effects of peripheral blood B lymphocytes(CD3^(-)CD19^(+)) and interleukin(IL) -6 on anemia in systemic lupus erythematosus(SLE), and explore the potential targets that can reduce the risk of anemia in SLE patients.Methods In this case-control study, the case data of 70 SLE patients with anemia treated in the Affiliated Hospital from March 2020 to February 2022 were collected and included in the occurrence group. In addition, the case data of 66 SLE patients without anemia admitted to the hospital in the same period were collected and included in the non occurrence group. The test indicators required for the study in the case data was checked. The admission examination items included T lymphocyte(CD4^(+)), CD3^(-)CD19^(+), serum IL-6, blood routine and other SLE related indicators. A logistic regression model was constructed according to the statistical results to analyze the effects of CD3^(-)CD19^(+)and serum IL-6 on anemia in SLE patients. Pearson correlation was used to analyze the relationship between CD3^(-)CD19^(+)and serum IL-6 in SLE patients.Results The SLEDAI score, CD4^(+), CD3^(-)CD19^(+)count and serum IL-6 expression in the occurrence group were higher than those in the non occurrence group, and the proportion of renal lesions was higher than that in the non occurrence group, the difference was statistically significant(P < 0.05). In other data, there was no statistical significant difference between the two groups(P>0.05). The logistic regression model was constructed. The high score of SLEDAI, the high expression of CD3^(-)CD19^(+), IL-6, and the renal lesion could increase the risk of anemia in SLE patients(OR>1, P < 0.05). Pearson correlation test showed that, there was a positive correlation between CD3^(-)CD19^(+)and IL-6 in peripheral blood of SLE patients(r>0, P < 0.05).Conclusion B cell and IL-6 were highly expressed in SLE patients with anemia. The high expression of B cell and IL-6, disease activity and renal disease could increase the risk of anemia in SLE patients.
