高压氧联合Schuell语言训练治疗脑卒中后失语症患者的疗效及其对语言中枢活动功能的影响

Efficacy of early hyperbaric oxygen therapy combined with Schuell language training on patients with aphasia after stroke and its influence on the activity function of language center

摘要 目的探讨高压氧联合Schuell语言训练治疗脑卒中后失语症患者的疗效及其对语言中枢活动功能的影响。方法选取徐州市中心医院康复科2021年3月至2022年3月收治的176例脑卒中后失语症患者为研究对象,采用随机数字表法分为对照组和观察组,每组88例。对照组患者采用Schuell语言训练治疗,观察组在此基础上联合高压氧治疗。分析2组患者的疗效;采用美国国立卫生研究院卒中量表(NIHSS)、蒙特利尔认知量表(MoCA)和波士顿诊断性失语症检查量表(BDAE),观察2组患者治疗前后的神经功能损伤程度、认知功能和语言中枢活动功能。结果所有疗程结束后,观察组患者的治疗总有效率为88.64%,明显高于对照组的73.86%,差异有统计学意义(P<0.05)。治疗前,2组患者的NIHSS、MoCA和语言中枢活动功能评分比较差异均无统计学意义(P>0.05);与治疗前相比,治疗后2组患者的NIHSS评分降低,MoCA和语言中枢活动功能评分升高,且观察组患者治疗后的NIHSS评分低于对照组,MoCA评分和各项语言中枢活动功能评分均高于对照组,差异有统计学意义(P<0.001)。结论高压氧治疗联合Schuell语言训练可以提高脑卒中后失语症患者的整体疗效,降低患者的神经功能损伤程度,提高其认知功能和语言中枢活动功能。 Objective To explore the efficacy of early hyperbaric oxygen therapy combined with Schuell language training on patients with aphasia after stroke,and to analyze its influence on the activity function of language center.Methods A total of 176 patients with aphasia after stroke admitted to the Department of Rehabilitation of Xuzhou Central Hospital from March 2021 to March 2022 were selected as research objects,and were divided into control group and observation group by random number table method,with 88 cases in each group.The control group received Schuell language training,while the observation group was additionally treated with hyperbaric oxygen therapy.The efficacy of the two groups were analyzed by observing the degree of neurological impairment,cognitive function,and language center activity function in both groups before and after treatment,with National Institutes of Health Stroke Scale(NIHSS),Montreal Cognitive Scale(MoCA),and Boston Diagnostic Aphasia Scale(BDAE).Results After all courses of treatment,the total effective treatment rate of the observation group was 88.64%(78/88),which was significantly higher than that of the control group(73.86%,65/88),and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in NIHSS,MoCA,and language center activity function score between the two groups(P>0.05).Compared with those before treatment,the NIHSS scores of the two groups were decreased,and the scores of MoCA and language center activity function were increased;furthermore,after treatment,the NIHSS score of the observation group was lower than that of the control group,and the scores of MoCA and various language center activity function were higher than those of the control group,with statistically significant differences(P<0.001).Conclusion Early hyperbaric oxygen therapy combined with Schuell language training can improve the overall efficacy on patients with aphasia after stroke,and reduce the degree of neurological impairment,thus improving the cognitive function and language center activity function.
