高压氧治疗对瘢痕疙瘩组织中瘙痒相关因子的影响

摘要 目的探讨高压氧治疗对瘢痕疙瘩组织中瘙痒相关因子表达的影响以及瘢痕疙瘩瘙痒症状的发生机制。方法选取2021年2月至8月北京协和医院整形美容外科收治的12例瘢痕疙瘩患者作为研究对象,按照治疗方法分为高压氧治疗组(HK组)和对照组(K组),每组6例。2组患者均行手术切除加放疗,HK组患者在术前接受4次高压氧治疗。采用免疫组化染色法半定量分析手术切除后所得胸部瘢痕疙瘩标本中缺氧诱导因子-1α(HIF-1α)、色氨酸羟化酶1(TPH1)、辣椒素受体1(TRPV1)、间隙连接蛋白43(Cx43)的表达水平。利用单调方程评价HIF-1α与TPH1、TRPV1、Cx43的相关性。结果与K组相比,HK组标本中HIF-1α、TPH1、Cx43的表达明显降低,差异有统计学意义(P<0.05);TRPV1的表达降低,但差异无统计学意义(P>0.05)。HIF-1α与Cx43、TRPV1呈中等正相关、与TPH1呈弱正相关,差异无统计学意义(P>0.05)。结论高压氧治疗可降低瘢痕疙瘩组织Cx43、TPH1、HIF-1α的表达水平。HIF-1α与Cx43、TRV1、TPH1呈正相关趋势,可能为瘢痕疙瘩瘙痒的调控分子之一。 Objective To explore the effect of hyperbaric oxygen therapy(HBOT)on pruritus-related factors in keloid tissue,and to further explore the possible mechanism of keloid pruritus.Methods The keloid specimens were collected postoperatively from 12 patients who visited the department of plastic surgery of Peking union medical college hospital from February to August 2021.The patients were divided into hyperbaric oxygen therapy(HK)group and control(K)group,with six cases in each group.Patients in both groups underwent surgical resection and radiotherapy.The HK group received four sessions of hyperbaric oxygen therapy prior to surgery.The expression levels of hypoxia-inducible factor-1α(HIF-1α),tryptophan hydroxylase 1(TPH1),transient receptor potential vanilloid 1(TRPV1),and gap junction protein 43(Cx43)in scar specimens obtained after surgical resection were analyzed semi-quantitatively using immunohistochemical staining.The correlations between HIF-1αand TPH1,TRPV1,Cx43,respectively,were evaluated using monotonic equations.Results Compared with K group,the expression levels of HIF-1α,TPH1,and Cx43 in the specimens of HK group were significantly decreased,and the differences were statistically significant(P<0.05).The expression level of TRPV1 in HK group was also decreased,but the difference was not statistically significant(P>0.05).HIF-1αshowed moderate positive correlations with Cx43 and TRPV1,and a weak positive correlation with TPH1,but the differences were not statistically significant(P>0.05).Conclusion Hyperbaric oxygen therapy can reduce the expression levels of Cx43,TPH1,and HIF-1αin keloid tissue.HIF-1αshows positive correlation trends with Cx43,TRPV1,and TPH1,indicating its potential role as a regulatory factor for keloid pruritus.
