低剂量螺旋CT扫描联合血清miR-940、miR-412对肺癌的诊断价值

摘要 目的探究低剂量螺旋CT扫描联合血清miR-940、miR-412对肺癌的诊断价值。方法回顾性分析我院2020年7月-2022年11月收治的96例肺癌患者(肺癌组)临床资料,另选取同期在我院诊治的96例肺部良性病变患者为对照组。对所有受试者进行低剂量螺旋CT扫描检查;实时荧光定量PCR法检测肺癌患者及对照组患者血清miR-940、miR-412表达水平;受试者工作特征(ROC)曲线评价血清miR-940、miR-412对肺癌的诊断价值;以病理学检查为金标准,分析低剂量螺旋CT扫描、血清miR-940、miR-412单独诊断及三者联合诊断肺癌的诊断效能。结果肺癌组患者血清miR-940水平(0.69±0.18)水平显著低于对照组患者(0.97±0.30)(P<0.05),miR-412水平(1.49±0.33)显著高于对照组患者(1.10±0.30)(P<0.05);低剂量螺旋CT扫描诊断肺癌的灵敏度为78.13%、特异度为73.96%,准确度为76.04%;ROC曲线结果显示,miR-940、miR-412单独诊断肺癌的AUC为0.782、0.799,敏感度分别为87.5%、62.5%,特异性分别为53.1%、56.2%,准确度分别为70.31%、59.38%,血清miR-940、miR-412联合诊断肺癌的AUC为0.880,敏感度为89.6%,特异性为68.8%;低剂量螺旋CT扫描联合血清miR-940、miR-412诊断肺癌的敏感度为91.67%,特异度为89.58%,准确度为90.63%,联合诊断效能显著高于低剂量螺旋CT扫描、血清miR-940、miR-412单独检测。结论低剂量螺旋CT扫描联合血清miR-940、miR-412诊断肺癌具有一定的诊断价值,可有效减小误诊率及漏诊率。 Objective To explore the diagnostic value of low-dose spiral CT scanning combined with serum miR-940 and miR-412 for lung cancer.Methods A retrospective analysis was conducted on the clinical data of 96 lung cancer patients(lung cancer group)admitted to our hospital from July 2020 to November 2022,another 96 patients with benign lung lesions who were diagnosed and treated in our hospital were collected as the control group.Low-dose spiral CT scan was performed on all subjects;realtime fluorescence quantitative PCR method was applied to detect the expression levels of miR-940 and miR-412 in the serum of patients in lung cancer and patients in control group;receiver operating characteristic(ROC)curve was applied to evaluate the diagnostic value of serum miR-940 and miR-412 for lung cancer;using pathological examination as the gold standard,the diagnostic efficacy of low-dose spiral CT scans,serum miR-940,miR-412 alone,and their combination in the diagnosis of lung cancer was analyzed.Results The serum miR-940 level(0.69±0.18)in the lung cancer group was obviously lower than that in the control group(0.97±0.30)(P<0.05),and the miR-412 level(1.49±0.33)was obviously higher than that in the control group(1.10±0.30)(P<0.05);the sensitivity,specificity,and accuracy of low-dose spiral CT scanning in the diagnosis of lung cancer were 78.13%,73.96%,and 76.04%,respectively;the ROC curve results showed that the AU C of miR-940 and miR-412 for diagnosing lung cancer alone was 0.782 and 0.799,with sensitivity of 87.5%and 62.5%,specificity of 53.1%and 56.2%,accuracy of 70.31%and 59.38%,respectively,the AU C of serum miR-940 combined miR-412 for diagnosing lung cancer was 0.880,with sensitivity of 89.6%and specificity of 68.8%;the sensitivity,specificity,and accuracy of low-dose spiral CT combined with serum miR-940 and miR-412 in the diagnosis of lung cancer were 91.67%,89.58%,and 90.63%,respectively,the combined diagnostic efficacy was obviously higher than that of low-dose spiral CT combined with serum miR-940 and miR-412 detection alone.Conclusion Low dose spiral CT scanning combined with serum miR-940 and miR-412 has certain diagnostic value in the diagnosis of lung cancer,which can effectively reduce the misdiagnosis rate and missed diagnosis rate.
