艾灸联合穴位注射治疗IgA肾病二联疗法致免疫力低下的临床疗效观察

Clinical observation of moxibustion combined with acupoint injection in the treatment of hypoimmunity caused by dual therapy of IgA nephropathy

目的:探讨艾灸联合穴位注射治疗IgA肾病二联疗法致免疫力低下的临床疗效。方法:选择2020年9月至2021年9月就诊的IgA肾病二联疗法致免疫力低下患者80例,按随机数字表法分为对照组和研究组各40例。对照组接受穴位注射治疗,经双侧三阴交、足三里穴注射重组人白介素-2,每穴20万IU,交替注射,1次/d,连续治疗4 d。在此基础上,研究组加用艾灸治疗,取双侧肾俞穴、命门穴,对准施灸部位实施熏灸,1次/d,15~20 min/次,连续14 d。比较两组治疗第14天时和治疗后第10天的T淋巴细胞亚群、免疫球蛋白和白细胞计数水平。结果:治疗后两组CD3^(+)、CD4^(+)、CD8^(+)均高于治疗前,治疗后第10天的CD3^(+)、CD4^(+)、CD8^(+)水平高于治疗第14天时,且研究组治疗第14天时和治疗后第10天的CD3^(+)、CD4^(+)、CD8^(+)水平均高于对照组(P<0.05)。治疗后两组白细胞、IgG、IgA水平均高于治疗前,治疗后第10天的白细胞、IgG、IgA水平均高于治疗第14天时。研究组治疗第14天和治疗后第10天的白细胞、IgG、IgA水平均高于对照组(P<0.05)。结论:艾灸联合穴位注射治疗可有效改善IgA肾病二联疗法致免疫力低下患者免疫功能,提高白细胞水平。

Objective:To investigate the clinical efficacy of moxibustion combined with acupoint injection in the treatment of hypoimmunity caused by dual therapy of IgA nephropathy.Methods:A total of 80 patients with hypoimmunity caused by dual therapy of IgA nephropathy from September 2020 to September 2021 were selected and divided into a control group and a treatment group with 40 patients in each group according to random number table method.The control group received acupoint injection treatment,and the recombinant human interleukin-2 was injected by bilateral Sanyinjiao and Zusanli points,200000 IU per point,alternately once a day,for continuous treatment for 4 days.On this basis,moxibustion was added to the treatment group,and moxibustion was carried out at both Shenshu and Mingmen points,once a day,15-20 min/time,for 14 consecutive days.The T lymphocyte subsets,immunoglobulin and white blood cell count levels were compared between the two groups on the 14th day of treatment and the 10th day after treatment.Results:After treatment,the levels of CD3^(+),CD4^(+)and CD8^(+)in both groups were higher than those before treatment,and the levels of CD3^(+),CD4^(+)and CD8^(+)on the 10th day after treatment were higher than those on the 14th day of treatment.The levels of CD3^(+),CD4^(+)and CD8^(+)in the treatment group were higher than those in the control group on the 14th day of treatment and on the 10th day after treatment(P<0.05).After treatment,the levels of WBC,IgG and IgA in both groups were higher than those before treatment,and the levels of WBC,IgG and IgA on the 10th day after treatment were higher than those on the 14th day of treatment,and the levels of WBC,IgG and IgA in the treatment group were higher than those in the control group on the 14th day of treatment and the 10th day after treatment(P<0.05).Conclusion:Moxibustion combined with acupoint injection can effectively improve the immune function and increase the level of WBC in patients with hypoimmunity caused by dual therapy of IgA nephropathy.
