中药熏洗结合功能锻炼对踝关节骨折术后疼痛、关节功能恢复的影响

The Influence of Chinese Medicinal Fumigation and Washing Combined with Functional Exercise on Postoperative Pain and Joint Function Recovery in Ankle Fracture Surgery

目的:观察中药熏洗结合功能锻炼对踝关节骨折术后疼痛、关节功能恢复的影响。方法:将2020年8月至2021年9月在常州市武进中医医院行踝关节骨折手术治疗的患者90例,按照随机数字表法分为参照组和研究组,每组各45例。两组患者均进行切开复位内固定手术,参照组术后给予功能锻炼治疗,研究组在参照组治疗的基础上给予中药熏洗治疗。运用疼痛视觉模拟评分(visual analogue score, VAS)、踝关节功能评分(American orthopaedic foot and ankle society, AOFAS)分别评估患者的疼痛程度、踝关节功能及临床疗效,并对比两组治疗前后踝关节主动活动度、并发症发生情况。结果:研究组治疗4周后、8周后VAS评分均低于参照组,差异有统计学意义(P<0.05)。研究组治疗后背伸角度、跖屈角度、AOFAS评分高于参照组,差异有统计学意义(P<0.05)。参照组优良率为77.78%,研究组优良率为95.56%,研究组优良率高于参照组,差异有统计学意义(P<0.05)。研究组并发症发生率为2.22%,参照组并发症发生率为17.78%,研究组低于参照组,差异有统计学意义(P<0.05)。结论:中药熏洗结合功能锻炼可减轻踝关节骨折患者术后疼痛感,提高踝关节主动活动度,促进踝关节功能恢复,并发症发生率低。

Objective:To observe the influence of Chinese medicinal fumigation and washing combined with functional exercise on post-operative pain and joint function recovery in ankle fracture surgery.Methods:A total of 90 patients who underwent ankle fracture surgery at Wujin Hospital of TCM in Changzhou City from August 2020 to September 2021 were randomly divided into the reference group and the study group,with 45 cases in each group.Both groups first underwent open reduction and internal fixation surgery.Then the refer-ence group received postoperative functional exercise treatment additionally,while the study group received Chinese medicinal fumiga-tion and washing treatment on the basis of the reference group.The visual analog score(VAS)and American orthopedic foot and ankle society(AOFAS)were used to evaluate the degree of pain,ankle function,and clinical efficacy of patients.The range of ankle motion and incidence of complications before and after treatment between the two groups were compared.Results:After 4 and 8 weeks of treat-ment,the VAS score of the study group was lower than that of the control group respectively,and all the differences were statistically sig-nificant(P<0.05).After treatment,the study group had higher back extension angle,plantar flexion angle,and AOFAS score than those of the control group,and all the differences were statistically significant(P<0.05).The excellent rate of the reference group was 77.78%,while that of the study group was 95.56%.The excellent rate of the study group was higher than that of the reference group,and the difference was statistically significant(P<0.05).The incidence of complications in the study group was 2.22%,which was lower than that of the reference group:17.78%,and the difference was statistically significant(P<0.05).Conclusion:Chinese medici-nal fumigation and washing combined with functional exercise can alleviate postoperative pain in patients with ankle fractures,improve active ankle mobility,promote ankle function recovery,and have a low incidence of complications.
