血清胱抑素C和尿微量白蛋白、尿肌酐联合检测在痛风性肾病早期诊断中的应用价值

Value of unite detection of serum cystatin C,microalbuminuria and urinary creatinine in early diagnosis of gouty nephropathy

目的探究血清胱抑素C和尿微量白蛋白、尿肌酐联合检测在痛风性肾病早期诊断中的应用价值。方法抽取2021年5月至2022年10月聊城市第三人民医院收治的50例痛风性肾病患者作为研究组,另抽取同期收治的50例普通痛风患者作为对照A组,50例参与健康体检的健康者作为对照B组。比较两组血清胱抑素C、尿微量白蛋白、尿肌酐水平及尿微量白蛋白/尿肌酐;比较各检测指标单项检测与联合检测诊断痛风性肾病的特异度、敏感度、精确率。结果研究组血清胱抑素C、尿微量白蛋白、尿肌酐水平及尿微量白蛋白/尿肌酐高于对照A组及对照B组(P<0.05);三组胱抑素C、尿微量白蛋白、尿肌酐联合检测的特异度、敏感度及精确率均高于各单项检测(P均<0.05)。结论血清胱抑素C、尿微量白蛋白、尿肌酐水平升高可作为早期痛风性肾病诊断的敏感指标。

Objective To investigate the value of unite detection of serum cystatin C,microalbuminuria and urinary creatinine in eraly diagnosis of gouty nephropathy.Methods Fifty gouty nephropathy patients admitted to Liaocheng Third People’s Hospital from May 2021 to October 2022 were selected as the study group;50 patients with simple gout treated in the hospital during the same period were selected as the control group A;and 50 healthy individuals who underwent physical examinations were selected as the control group B.The levels of cystatin C,microalbuminuria,urinary creatinine,and microalbuminuria/urinary creatinine were compared between two groups.The specificity,sensitivity and accuracy of cystatin C,microalbuminuria,urinary creatinine and combination of them in diagnosis of gouty nephropathy were compared.Results The levels of cystatin C,microalbuminuria,urinary creatinine,and microalbuminuria/urinary creatinine in the study group were higher than those in the control group A and control group B(P<0.05).The specificity,sensitivity and accuracy of combination of cystatin C,microalbuminuria and urinary creatinine were significantly higher than those of the 3 single indicators detection in the three groups(all P<0.05).Conclusions The increase of levels of cystatin C,microalbuminuria and urinary creatinine can be used as sensitive indicators for the diagnosis of early gouty nephropathy.
