摘要
目的比较经鼻高流量氧疗(HFNC)与经鼻持续气道正压通气(NCPAP)在新生儿呼吸窘迫综合征(NRDS)中的治疗效果。方法选取2017年1月至2020年12月南宁市妇幼保健院新生儿诊疗中心137例接受气管插管-肺表面活性物质-拔管技术治疗的NRDS早产儿,根据不同治疗方法分为NCPAP组(65例)和HFNC组(72例)。NCPAP组予以NCPAP,HFNC组予以HFNC。比较两组血气指标、呼吸功能指标、无创通气失败发生情况及并发症发生情况。结果NCPAP组动脉血二氧化碳分压、呼吸功能指标及无创通气失败发生率均优于HFNC组(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论NCPAP在NRDS中的治疗效果优于HFNC,可有效改善患儿血气指标、呼吸功能,降低无创通气失败发生率,值得临床推广。
Objective To compare the therapeutic effect of high flow nasal cannula oxygen therapy(HFNC)and nasal continuous positive airway pressure(NCPAP)in neonatal respiratory distress syndrome(NRDS).Methods From January 2017 to December 2020,137 premature infants with NRDS who were treated with intubation-surfactant-extubation technique in Neonatal Medical Center,Nanning Maternity and Child Health Hospital were divided into NCPAP group(65 cases)and HFNC group(72 cases)according to different treatment methods.NCPAP group was given NCPAP and HFNC group was given HFNC.Blood gas indexes,respiratory function indexes,occurrence of non-invasive ventilation failure and complications were compared between the two groups.Results The arterial partial pressure of carbon dioxide,respiratory function indexes and non-invasive ventilation failure rate in NCPAP group were better than those in HFNC group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The therapeutic effect of NCPAP in NRDS is better than that of HFNC,which can improve blood gas indexes and respiratory function of children effectively,and reduce the incidence of non-invasive ventilation failure,which is worthy of clinical promotion.
