子宫动脉血流参数联合可溶性血管内皮生长因子受体1、胎盘生长因子及可溶性内皮因子水平在子痫前期中的预测价值

Predictive value of uterine artery blood flow parameters combined with soluble vascular endothelial growth factor receptor-1,placental growth factor and soluble endothelial factor levels in preeclampsia

目的探讨子宫动脉血流参数联合可溶性血管内皮生长因子受体1(sFIt-1)、胎盘生长因子(PLGF)及可溶性内皮因子(sEng)水平在子痫前期中的预测价值。方法选取2022年1月至11月于复旦大学附属中山医院青浦分院进行产前检查并确诊为子痫前期的72例孕妇作为观察组,选取同期72例正常孕妇作为对照组,对比两组孕妇的子宫动脉血流参数、sFIt-1、PLGF及sEng指标,并分析相关指标在子痫前期早期预测中的价值。结果观察组孕妇血清sFIt-1、PLGF水平低于对照组,sEng水平高于对照组(P<0.05);观察组孕妇子宫动脉搏动指数、阻力指数、脐动脉收缩末期峰值/舒张末期峰值均高于对照组(P<0.05);子宫动脉血流参数、sFIt-1、PLGF及sEng受试者工作特征曲线显示,联合检测的敏感度为91.05%,特异度为94.07%,曲线下面积为0.8845。结论子宫动脉血流参数联合sFIt-1、PLGF及sEng水平在子痫前期中具有较高的预测价值。

Objective To explore the predictive value of uterine artery blood flow parameters combined with soluble vascular endothelial growth factor receptor-1(sFIT-1),placental growth factor(PLGF)and soluble endothelial factor(sEng)levels in preeclampsia.Methods A total of 72 gravidas who underwent prenatal examination and were diagnosed with preeclampsia in Qingpu Branch of Zhongshan Hospital Affiliated to Fudan University from January to November 2022 were selected as the observation group,and 72 normal gravidas during the same period were selected as the control group.The uterine artery blood flow parameters,sFlt-1,PLGF,and sEng levels were compared between the two groups,and the value of these indicators in early prediction of preeclampsia was analyzed.Results The serum levels of sFlt-1 and PLGF in the observation group were lower than those in the control group,and the level of sEng in the observation group was higher than that in the control group(P<0.05).The uterine artery pulsatility index,uterine artery resistance index,umbilical artery peak end-systolic velocity/peak end-diastolic velocity in the observation group were higher than those in the control group(P<0.05).The receiver operating characteristic curve of uterine artery blood flow parameters,sFlt-1,PLGF,and sEng showed that the sensitivity and specificity of the combined detection were 91.05%,94.07%,and the area under the curve was 0.8845.Conclusion Uterine arterial blood flow parameters combined with sFIT-1,PLGF,and sEng levels has high predictive value in preeclampsia.
