持续被动活动仪联合康复治疗在下肢骨折患者中的应用效果

摘要 目的:观察持续被动活动仪联合康复治疗在下肢骨折术后患者中的应用效果。方法:回顾性分析2019年1月至2021年1月该院收治的102例下肢骨折术后患者的临床资料,按照治疗方法不同将其分为对照组和观察组各51例。对照组实施康复治疗,观察组在对照组基础上联合持续被动活动仪治疗,两组均治疗8周。比较两组临床疗效,治疗前后疼痛[视觉模拟评分法(VAS)]评分、骨代谢指标[碱性磷酸酶(ALP)、24 h尿羟脯氨酸(HYP)]水平和生命质量[健康状况调查简表(SF-36)]评分。结果:观察组治疗总有效率为98.04%(50/51),高于对照组的82.35%(42/51),差异有统计学意义(P<0.05);治疗后,观察组VAS评分低于对照组,差异有统计学意义(P<0.05);治疗4、8周后,观察组ALP、HYP水平均高于对照组,差异有统计学意义(P<0.05);治疗后,观察组生理职能、社会功能、精力、生理功能、精神健康、一般健康状况、躯体疼痛、情感职能等SF-36各维度评分均高于对照组,差异有统计学意义(P<0.05)。结论:持续被动活动仪联合康复治疗应用于下肢骨折术后患者可提高治疗总有效率、骨代谢指标水平和生命质量评分,降低疼痛评分,效果优于单纯康复治疗。 Objective:To observe application effects of continuous passive motion combined with rehabilitation therapy in postoperative patients with lower limb fractures.Methods:The clinical data of 102 patients with lower limb fractures treated in this hospital from January 2019 to January 2021 were retrospectively analyzed.According to different treatment methods,they were divided into control group and observation group,51 cases in each group.The control group was treated with rehabilitation therapy,while the observation group was treated with continuous passive motion on the basis of the control group.Both groups were treated for 8 weeks.The clinical efficacy,the pain[visual analogue scale(VAS)]score,the bone metabolism index[alkaline phosphatase(ALP),24 h urinary hydroxyproline(HYP)]levels,and the quality of life[short form 36 health survey(SF-36)]score were compared between the two groups before and after the treatment.Results:The total effective rate of the observation group was 98.04%(50/51),which was higher than 82.35%(42/51)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the VAS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).After 4 and 8 weeks of treatment,the levels of ALP and HYP in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,after the treatment,the scores of SF-36 dimensions such as physiological role,social function,vitality physiological function,mental health,general health,physical pain and emotional function in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:Continuous passive motion combined with rehabilitation therapy can improve the total effective rate of treatment,the bone metabolism index levels and the quality of life scores,and reduce the pain scores in the postoperative patients with lower limb fractures.Moreover,it is superior to single rehabilitation therapy.
