彩色多普勒超声心动图在心肌梗死并发室壁瘤诊断中的应用价值

Application value of color Doppler echocardiography in diagnosis of myocardial infarction complicated with ventricular aneurysm

目的:探讨彩色多普勒超声心动图在心肌梗死并发室壁瘤诊断中的应用价值。方法:选取2020年1月至2022年12月该院收治的89例疑似心肌梗死并发室壁瘤患者进行前瞻性研究。所有患者均行常规心电图、彩色多普勒超声心动图检查,以左心室造影结果为金标准,统计两种检查方法室壁瘤检出情况,比较两种检查方法诊断心肌梗死并发室壁瘤的效能及与金标准的一致性。结果:89例疑似患者中,经左心室造影确诊72例,其中常规心电图检出66例,检出率为91.67%(66/72);彩色多普勒超声心动图检出71例,检出率为98.61%(71/72);彩色多普勒超声心动图检查诊断心肌梗死并发室壁瘤的灵敏度、准确度均高于常规心电图,差异有统计学意义(P<0.05);Kappa检验结果显示,常规心电图诊断心肌梗死并发室壁瘤与金标准的一致性尚可(Kappa值=0.680,P=0.000),彩色多普勒超声心动图诊断心肌梗死并发室壁瘤与金标准的一致性极好(Kappa值=0.893,P=0.000)。结论:彩色多普勒超声心动图检查诊断心肌梗死并发室壁瘤的效能高于常规心电图检查。

Objective:To explore application value of color Doppler echocardiography in diagnosis of myocardial infarction complicated with ventricular aneurysm.Methods:A prospective study was conducted on 89 patients with suspected myocardial infarction complicated with ventricular aneurysm admitted to the hospital from January 2020 to December 2022.All patients underwent routine electrocardiogram and color Doppler echocardiography.Using the results of left ventricular angiography as the gold standard,the detection of ventricular aneurysm by the two methods was statistically analyzed.The efficiencies of the two methods in the diagnosis of myocardial infarction complicated with ventricular aneurysm and the consistencies with the gold standard were compared.Results:Among the 89 suspected patients,72 patients were diagnosed by left ventricular angiography,of which 66 patients were detected by routine electrocardiogram with a detection rate of 91.67%(66/72),71 patients were detected by color Doppler echocardiography with a detection rate was 98.61%(71/72).The sensitivity and the accuracy of color Doppler echocardiography in the diagnosis of myocardial infarction complicated with ventricular aneurysm were higher than those of routine electrocardiogram,and the differences were statistically significant(P<0.05).Kappa test showed that the consistency of routine electrocardiogram in the diagnosis of myocardial infarction complicated with ventricular aneurysm with the gold standard was good(Kappa value=0.680,P=0.000),and the consistency of color Doppler echocardiography with the gold standard was extremely high(Kappa value=0.893,P=0.000).Conclusions:The efficiency of color Doppler echocardiography in the diagnosis of myocardial infarction complicated with ventricular aneurysm is higher than that of routine electrocardiogram.
