血清BAFF、Nesfatin-1、NLRP3水平与癫痫患者预后的相关性及其预测价值

Correlations among serum BAFF,Nesfatin-1 and NLRP3 levels and prognosis of patients with epilepsy and their predictive value

目的:分析血清B细胞活化因子(BAFF)、新饱食分子蛋白1(Nesfatin-1)、NOD样受体家族含pyrin结构域蛋白3(NLRP3)水平与癫痫患者预后的相关性及其预测价值。方法:选取2019年6月至2021年6月该院收治的149例癫痫患者进行前瞻性研究。治疗后随访1年,根据1年后预后情况将患者分为预后不良组(n=43)和预后良好组(n=106)。比较两组治疗前及治疗3、6个月后血清BAFF、Nesfatin-1、NLRP3水平,采用Spearman相关性分析BAFF、Nesfatin-1、NLRP3水平与癫痫患者预后的相关性,并采用受试者工作特征(ROC)曲线分析血清BAFF、Nesfatin-1、NLRP3水平单项及联合检测预测癫痫患者预后的价值。结果:治疗3、6个月后,预后不良组血清BAFF、Nesfatin-1、NLRP3水平均高于预后良好组,差异有统计学意义(P<0.05);Spearman相关性分析结果显示,BAFF、Nesfatin-1、NLRP3水平与癫痫患者预后均呈正相关(r>0,P<0.05);治疗6个月后,血清BAFF、Nesfatin-1、NLRP3水平单项及联合预测癫痫患者预后的曲线下面积(AUC)分别为0.730、0.756、0.737、0.906,均具有一定预测价值,其中联合检测预测价值最高。结论:血清BAFF、Nesfatin-1、NLRP3水平与癫痫患者预后均呈正相关,且三者联合检测预测癫痫患者的价值高于各单项检测。

Objective:To analyze correlations among serum B cell activating factor(BAFF),Nesfatin-1,NOD-like receptor family pyrin domain containing protein 3(NLRP3)levels and prognosis of patients with epilepsy and their predictive value.Methods:A prospective study was conducted on 149 patients with epilepsy admitted to the hospital from June 2019 to June 2021.These patients were followed up for 1 year after the treatment.According to the prognosis after 1 year,these patients were divided into poor prognosis group(n=43)and good prognosis group(n=106).The levels of serum BAFF,Nesfatin-1 and NLRP3 were compared between the two groups before and 3,6 months after the treatment.Spearman correlation analysis was used to analyze the correlations among the BAFF,Nesfatin-1,NLRP3 levels and prognosis.The receiver operating characteristic(ROC)curve was used to analyze the value of single and combined detection of serum BAFF,Nesfatin-1 and NLRP3 levels in the prediction of the prognosis of the patients with epilepsy.Results:After 3 and 6 months of treatment,the levels of serum BAFF,Nesfatin-1 and NLRP3 in the poor prognosis group were higher than those in the good prognosis group,and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that the levels of BAFF,Nesfatin-1,and NLRP3 were positively correlated with the prognosis of the patients with epilepsy(r>0,P<0.05).After 6 months of treatment,the AUCs of single and combined detection of serum BAFF,Nesfatin-1 and NLRP3 levels in the prediction of the prognosis of the patients with epilepsy were 0.730,0.756,0.737,and 0.906,separately,they all had certain diagnostic efficiencies,and the combined detection had the highest prediction efficiency.Conclusions:The serum BAFF,Nesfatin-1 and NLRP3 levels are positively correlated with the prognosis of the patients with epilepsy,and the value of combined detection of them in the prediction of the prognosis of the patients with epilepsy is higher than that of single detection of the three.
