基于信息不对称理论的临床路径变异率创新探析

Revision of clinical pathway variation rate based on information asymmetry theory

目的 为规避临床路径实施过程中,信息不对称所带来的医务人员道德风险,提出创新变异率的质量管理对策。方法 运用信息不对称理论,对当前临床路径质量管理过程中存在的问题进行分析。结果 先将变异率拆分为路径内和路径外医嘱变异率两个二级指标,又用路径内医嘱符合率来替代路径内医嘱变异率,对变异进行考核。结论 变异率的创新指标操作简单易行,较为客观实用,具备可复制、可推广的特点,可在全疆推广应用。

Objective In order to reduce the ethical risks of medical staff caused by information asymmetry in the process of clinical pathway operation,this paper proposes a quality management strategy for variation rate.Methods Using information asymmetry theory,this paper analyzed the problems existing in the current quality management process of clinical pathways.Results The indicators contained in the variation rate was first reduced into two sub-indicators:the variation rate of orders within and outside the pathway,and then the compliance rate of medical orders within the pathway was used to replace the variation rate of medical orders within the pathway.Finally the variation was assessed.Conclusion The revision of the indicator system of variation rate is simple,easy,reproducible,relatively objective and practical.Therefore,it can be spread within the whole Xinjiang region.
